Reporting to the Community Outreach Supervisor, the
Community Outreach Social Worke
r is a member of a multidisciplinary outreach team that aims to meet the social determinants of health for community members experiencing multiple barriers, which can result in chronic homelessness and/or increased utilization of emergency health services. The Social Worker will utilize clinical skills within the outreach program that provides outreach addictions services to vulnerable clients experiencing homelessness or on the verge of homelessness who are not able to access conventional services. The Community Outreach Social Worker will utilize various approaches to meet the needs of individual clients by providing individual case consultation. There is a strong Mental Health component to this position as well. The Community Outreach Social Worker will provide assessment, counselling, and referrals. They will work in collaboration with outside community agencies and respond to emergency situations as needed.

DUTIES AND RESPONSIBILITIES:
To provide training opportunities for staff that will assist them in helping to move clients forward in dealing with their substance dependency
Provides assessment of clients' mental health and substance use needs, followed by referrals to receive diagnosis and treatment
Provides direct client care in relation to substance use disorders and mental health
Provides supportive counselling related to mental health and substance use disorders
Help facilitate the clients' optimized utilization of all available community supports
Provides ongoing assessment and supportive counselling to clients as needed
Provides group intervention to address substance use disorders and mental health using DBT and other methodologies
Works collaboratively within the multidisciplinary team to provide comprehensive case management to clients as needed
Document client interactions, client referrals and client progress on the Homeless Management Information System (HMIS), and the clinic's electronic medical record (EMR)
Facilitates emergency interventions, as needed
Develops and maintains effective working relationships within The Mustard Seed and with external agencies
Delivers presentations to external organizations, equipping their staff with the knowledge and tools to identify and support clients facing substance use disordersBuilds a comprehensive library & knowledge based on community resources related to substance dependency
Tracks statistics and prepares reports, as required
Assists the Community outreach supervisor and the Health and Wellness Manager with new initiatives or projects as required

MINIMUM QUALIFICATIONS AND SKILLS
Bachelor's degree in social work, with active registration in good standing with the Alberta College of Social Workers (ACSW) - strongly preferred
Demonstrated passion for contributing to the effort to end homelessness in Alberta
Minimum of two years' experience in the mental health field is considered an asset
Direct experience working with vulnerable populations, including individuals experiencing homelessness, is a significant advantage
Proven ability to conduct mental health assessments, provide counseling, and implement effective care plans
Strong verbal and written communication skills, with the ability to collaborate effectively with clients, multidisciplinary teams, and community agencies
Comprehensive knowledge of Calgary's mental health and addiction resources, with the ability to navigate those systems confidently
Understanding of FOIP legislation and adherence to confidentiality and privacy standards
Self-motivated, with the ability to work both independently and collaboratively within a cross-functional team
Proficient in Microsoft Office Suite; experience with the Homeless Management Information System (HMIS) is an asset
Strong organizational, time management, and advocacy skills
Excellent interpersonal, oral, and written communication skills
Valid Class 5 Driver's License and access to a reliable vehicle
WORKING CONDITIONS
This position involves a combination of office-based work and street outreach activities.
Duties will require lifting, traveling, sitting, standing, walking, and operating a personal vehicle.
Moderate physical effort is required to perform essential job functions, including data entry, navigating various environments, and responding effectively in a fast-paced setting
Adequate mental effort is needed for tasks such as counseling, providing referrals, conducting training, and managing sensitive or high-intensity situations.
Employees are to be aware that many of our clients may experience difficulties or impairment at times, which could result in them uttering threats, discriminatory statements, and other insults at your person. This may in a raised voice, or in seeing unpredictable behaviour up to and including the possibility of them becoming aggressive.
+ Training on how to respond is provided, but this is to be considered typical working conditions. Many of our clients are unaware of the impact of these comments.

This position requires the use of a company vehicle, and/or personal vehicle for company use. The candidate must have a valid driver's licence, maintain $2M Vehicle Liability Insurance on the personal vehicle being used, and will need to consent to a driver's abstract request.
