Do you want to be part of a project that empowers people living with dementia to lead, connect, and belong in their communities? One that centers Indigenous voices and works alongside partners to reduce isolation and foster inclusion?
The Hamilton Council on Aging (HCoA) is home to the Dementia Empowerment Network (DEN), a growing movement of peer-led groups created by and for people living with dementia, as well as those supporting their journey (care partners). These groups reduce social isolation, build friendships, and provide meaningful opportunities for leadership and connection.
Through a new partnership with Six Nations of the Grand River and De dwa da dehs nye>s Aboriginal Health Centre, we are expanding this model to support Indigenous older adults living with dementia and their care partners.
