About the role
As a
Community Nurse
for the Community Withdrawal Management Services (CWMS) program you will provide high-quality services to those living with addiction and substance use disorders. Under the supervision of a clinical supervisor, the work will be conducted in the community; visiting clients in their homes, at other organizations, in PAARC's office locations, or at mutually agreeable and safe locations within our geographic area, in person and using virtual platforms. PAARC offers a person-centred philosophy, including a harm reduction approach, to a wide range of client populations, and partners with many other agencies to provide integrated care.

What you'll do
Receive and process referrals from the Intake team, individual requests and from other sources for the CWMS program
Conduct nursing assessments using appropriate tools and methods, as determined by the addictions and mental health sectors and by partners as appropriate
Contribute to the preparation and modification of treatment plans based on the assessment of the client's conditions.
Engage in individual and group sessions with clients, to offer health promotion/consultation and support the achievement of health-related goals. Topics of particular focus include the management of physical withdrawal symptoms and navigating medical options/treatments of substance use disorders.
Provide nursing support and information to Counsellors/Case Managers on the CWMS and other teams
Conduct case conferences/consultation with treatment providers and other community organizations
Initiate a client file and maintain written and electronic documentation, according to best practice and legal guidelines, of all contacts with or about the client, in a timely manner, according to PAARC policy and guidelines
Provide recommendations or referrals to appropriate medical care when needed
Support the Smoking Cessation program including the management and dispensing of NRT products to clients
Represent PAARC as requested at educational and other events
Assist in the ongoing development, operation and evaluation of the program as appropriate
Skills & Competencies
Knowledge of all nursing practices according to standards of the College of Nurses of Ontario
Knowledge of opiate replacement therapies and the role they play in optimizing wellness
Experience in delivering health promotion/consultation through individual sessions, group facilitation and/or educational workshops
Able to engage in research and development related to nursing practices within PAARC programs
Excellent communication skills, both verbal and written, and the ability to work empathetically with people from all socio-economic backgrounds, including marginalized/stigmatized populations
Critical thinking skills and the capacity to work effectively as part of a multidisciplinary team.
Ability to work as an integral part in a multidisciplinary team
Requirements
Designation: Registered Practical Nurse (RPN) or Registered Nurse (RN) certification and active registration in good standing with the College of Nurses of Ontario
Experience: 1-3 years of nursing experience in a community based setting, including home visits
Experience working with clients with addictions, substance abuse, concurrent disorders, mental health and/or other vulnerable populations is an asset
Certifications: Current CPR and First Aid required. Non-violent Crisis intervention skills/certification an asset
Criminal Record Check (Vulnerable Sector) required
Language Skills: Proficiency in prevalent languages in the Region of Peel and French considered a strong asset.
Transportation: Valid Ontario Driver's License, insurance, and reliable vehicle required.
Working Conditions
Office environment and community-based settings.
35-hour work week; Mon, Tues, Thurs Fri 8:30 am- 4:30 pm, Wed 12:30pm-8pm
Required travel to clients homes and community facilities.

The pay range for this role is:
51,308 - 57,748 CAD per year(Argentia)
