The Community Navigator plays a vital role in connecting individuals and families with essential resources and services within the community. This position requires a compassionate and proactive individual who can effectively engage with diverse populations, assess their needs, and provide guidance to help them navigate available support systems. The Community Navigator will work collaboratively with local organizations, service providers, and community members to foster a supportive environment that promotes well-being and empowerment. Duties



Establish and maintain relationships with community organizations, service providers, and stakeholders to enhance resource accessibility. Conduct outreach activities to identify individuals and families in need of support services. Assist participants in understanding available resources and help them navigate the application processes for various programs. Provide one-on-one support to participants, including assessments of needs and developing personalized action plans. Maintain accurate records of participant interactions, referrals, and outcomes to track progress and effectiveness of services. Contribute to a collaborative team environment, helping where needed, while working independently in a fast-paced setting.

Experience

A degree in Social Work, Psychology, or a related field. Previous experience in community outreach, social services, or a related field is preferred. Strong interpersonal skills with the ability to communicate effectively with diverse populations. Proficiency in problem-solving and critical thinking skills to assess participant needs accurately. Familiarity with local resources, social services, and community programs is advantageous. Ability to work independently as well as part of a team in a dynamic environment. Job Types: Part-time, Fixed term contract
Contract length: 12 months

Pay: $22.00-$24.00 per hour

Expected hours: No less than 15 per week
