Duties
Establish and maintain relationships with community organizations, service providers, and stakeholders to enhance resource accessibility.
Conduct outreach activities to identify individuals and families in need of support services.
Assist participants in understanding available resources and help them navigate the application processes for various programs.
Provide one-on-one support to participants, including assessments of needs and developing personalized action plans.
Maintain accurate records of participant interactions, referrals, and outcomes to track progress and effectiveness of services.
Contribute to a collaborative team environment, helping where needed, while working independently in a fast-paced setting.
Experience
A degree in Social Work, Psychology, or a related field.
Previous experience in community outreach, social services, or a related field is preferred.
Strong interpersonal skills with the ability to communicate effectively with diverse populations.
Proficiency in problem-solving and critical thinking skills to assess participant needs accurately.
Familiarity with local resources, social services, and community programs is advantageous.
