About the Role
Reporting to the Director of Programs, the Community Navigator serves as a key bridge between clients and the network of housing, social service, and community resources. This position helps clients assess their housing needs, navigate available supports, and connect with community partners to remove barriers and achieve stable, accessible housing.
Key Responsibilities
Respond to program and service inquiries from clients and community partners.
Assess client needs related to housing accessibility, mobility, and supportive living.
Provide direct support, advocacy, and referrals to appropriate community and housing services.
Build and maintain relationships with service providers and partner agencies.
Monitor client progress, document outcomes, and ensure consistent follow-up.
Collaborate with internal teams to deliver coordinated, client-centered service.
Represent Accessible Housing at community meetings, trade shows, and presentations to promote awareness of housing accessibility and mobility challenges.
Contribute to program improvement by identifying trends, challenges, and opportunities for innovation.
Qualifications
Diploma or degree in Human Services or a related field (or equivalent combination of education and experience).
Minimum two years of related experience in community services, housing support, or case management.
Knowledge of local housing systems, social service networks, and accessibility supports.
Strong interpersonal, communication, and relationship-building skills.
Excellent organizational and time management abilities.
Proficiency with Microsoft Office; experience with database or case management systems is an asset.
Self-motivated and able to work both independently and collaboratively.
Commitment to confidentiality, ethical practice, and client dignity.
Satisfactory Criminal Record Check required.
