Job Overview


The Sara Riel Inc. is searching for an individual with strong interpersonal, communication and social skills to fill up the position of Community Mental Health/Peer Support Worker. The position is responsible in providing Rehabilitation, Recovery and Skill Development.

Represent the Mission, Vision and Values

of Sara Riel Inc.



Respond to requests for information and orient new and potential Service Recipients and referral sources to Sara Riel Inc. admission requirements and program guidelines.

Demonstrate a positive role model in all internal and external interactions, maintaining rapport with referral sources, to build a strong network of external resources, and facilitate intake.

Prepare for, attend and participate in all appointments, staff, team and case meetings and attend training, conferences and/or expositions as a representative of Sara Riel Inc.

Provide Support and Services with Administrative Related Duties

Provide support and services under a person-centered, recovery care model in accordance with to the self-identified needs of the Service Recipient.

Document all relevant information and generate clear, concise and accurate reports detailing significant interactions; and provides oral summarizations at shift changes; gather and record all statistical information necessary for program delivery and evaluation.

Other duties as assigned and in accordance with outlined policies, procedures and guidelines.

Ensure the Well-Being and Safety of the Service Recipients and the Property

Perform routine walk-throughs of the property; identify and take action to correct or minimize any safety risks; communicate any maintenance needs to the Supervisor or designate.

Undertake any necessary daily cleaning tasks specified in accordance with property standards.

Effectively Manage Crisis Situations

Identify and evaluate immediate risks of self-harm and/or violence and act to minimize these risks in accordance with Non-Violent Crisis Intervention Training (NVCI) and Applied Suicide Intervention Skills Training (ASIST).

Promote responsibility, empowerment and exploration of choices; facilitate problem-solving and communication to resolve the crisis and to minimize risk.

Notify Sara Riel Inc. On-Call, as well as notify/activate any emergency resources as necessary.

Document all relevant information and follow up with Sara Riel support team members.

Complete and submit Critical Incident Report forms to supervisor or designate within 24 hours.

COMMUNITY MENTOR SUPPORT



Rehabilitation and Recovery



Work with Service Recipient (and assigned staff) to determine individualized Rehabilitation and Recovery Plans, including SMART Goals and Action Plans focused on successful attainment.

Communicating with service teams and attending case conferences as needed.

Provide motivational support and assistance with time/daily schedule management through weekly one-on-one meetings, or as directed by Service Recipient's needs.

Support Service Recipient's medication compliance, as determined by their Plans.

Skill Development



Assist Service Recipients with improving and maintaining Instrumental Activities of Daily Living (IADLs) through instruction, role modeling and community mentorship which provide the skills and strategies necessary to successfully engage in their own mental health recovery process.

Develop outcome measures and evaluation tools directly demonstrate program effectiveness.

Monitor ongoing assessments to keep current, and approve changes made as required based on Service Recipient feedback, progress, and outcome measures.

Assist in the acquisition of additional networks of support and services.

Medication Management and Education; understanding purpose and use of medication.

Event Participation



Participate in initiatives for Sara Riel functions/events at least once per year.

Support could in include planning, development, coordination and general participation with events.

Company events may take place both during and outside regular working hours. Employees participating in an event outside of working hours will be permitted to flex their time.

QUALIFICATIONS AND ABILITIES


Degree, Diploma or Certification in a mental health and/or social services discipline.

2-5 years' experience in the provision of mental health and/or social services.

Strong written and verbal communications skills in English and French.

Satisfactory Employment record.

Ability to function in a demanding and stressful environment.

Demonstrated ability to work with minimal supervision.

Provide a current and satisfactory Criminal Record Check and Adult and Child Abuse Registry Check.

Experience in computer programs i.e., Microsoft Office, Outlook.

Understanding of mental health issues.

Good critical thinking and problem-solving skills required.

Acute sense of discretion, confidentiality and professionalism.

Ability to work effectively both independently and as part of a team.

Pay: $16.00 per hour
