HOMES Program Mobile Teams
How You Will Make an Impact
The HOMES program provides housing and supports to people living with mental health issues who have experienced homelessness or are at risk of being homeless. Team members support tenants in scattered apartment units throughout Hamilton to reach personal goals and to have a satisfying and meaningful life in the community.
Responsible for assessment, planning, crisis prevention and intervention, supportive counselling, education, advocacy, conflict resolution between tenants and landlords
Provide tenants with primary case management and community resources
Facilitating social interaction and community integration
Provide linkages with other community services
Some opportunities include work done in the community and in tenant apartments, with constant driving required for Mobile Teams
Demonstrate commitment to the Good Shepherd mission values of Hospitality, Responsibility, Availability, Adaptability, Quality and Dignity
Responsible for participating in Good Shepherd's Health and Safety programs, procedures, procedures and work in compliance with the provisions of the Occupational Health and Safety Act and its regulations to minimize risk to clients, staff and others as well as to the property of Good Shepherd
What You'll Bring to Our Team
A post-secondary education in social or health services
A minimum of three years' experience in mental health in a community-based setting
'Recovery orientation' and 'consumer driven' style of practice is essential
Possess the ability to work independently and think creatively
Ability to speak French and/or other languages considered an asset
CPR certification or willingness to be certified is required
Committed to anti-racist/anti-oppressive practice and to the mission and values of Good Shepherd
Possess full driving license as driving agency vehicles and personal vehicles is required for this position
