SHIP is presently looking for a Community Mental Health Counsellor who will provide treatment and support with system transitions to clients experiencing mental health challenges and addictions in the Dufferin community.

Duties and Responsibilities

  • Formulate an implement client-centered service plans
  • Provide time limited, short term, case management
  • Review referrals and conduct clinical assessments
  • Provide education on mental illness and crisis management
  • Develop follow-up plans as necessary
  • Connect clients to longer term service, if required
  • Refer to partnering organizations and facilitate linkages to community supports
  • Provide streamlined transitions with \xe2\x80\x9cwarm transfers\xe2\x80\x9d
  • Partner with clients and their families as members of the health care team
  • Liaise with community supports (i.e. mental health and addiction service providers, and family members) as required
Qualifications
  • Post-secondary education in Social Work, Health Services, or equivalent
  • 3+ years experience supporting individuals with mental health challenges in the community
  • Any experience providing DBT treatment is an asset
  • Certification with the Ontario College of Social Workers is an asset
  • Must have valid drivers license and access to a reliable vehicle
Working Conditions and Physical Requirements
  • Day shifts (Monday \xe2\x80\x93 Friday) with the ability to work excess/overtime hours where required
  • Travel by vehicle to community sites for department/program support as required
