Registered Nurse (RN) - Community Hospice Palliative Care Team


(designated bilingual position)



Part-time, Contract expires March 2026, 30 hours biweekly, Days 8:30-4:30



1st week: Sunday and Thursday, 2nd week: Monday and Saturday



Must be available to work holidays

POSITION SUMMARY


As a member of the Community Hospice Palliative Care Team (CHPCT), the Registered Nurse primarily provides

telephone-based support and consultation

to clients with life-limiting illnesses and their caregivers. This role focuses on holistic coordination of symptom management during the final year of life.

MAJOR AREAS OF RESPONSIBILITY


Conducts comprehensive telephone assessments using a holistic approach to address pain and symptoms across various illness trajectories and to support clients in achieving their goals of care.

Develops individualized care plans and monitors client progress in accordance with Maison McCulloch Hospice (MMH) policies and procedures.

Communicates findings to the Most Responsible Provider (MRP), which may be a physician or nurse practitioner.

Coordinates, schedules, and may attend home visits with the MRP to ensure continuity of care.

Collaborates with external partners, including the Northeast Cancer Centre, Ontario Health at Home, and contracted nursing providers.

Following additional training, may be assigned to provide direct resident care at MMH, including coverage of residential shifts during weekday staffing shortages (Monday to Friday, 8:30 a.m.-4:30 p.m.).

POSITION REQUIREMENTS



Necessary:



Bachelor of Science in Nursing (BScN)

Member of the College of Nurses of Ontario (CNO) in good standing

Bilingualism (English/French), written and verbal

Minimum of two years full-time equivalent experience providing care to individuals with life-limiting illness, plus three years of general nursing experience

Strong communication skills, with the ability to effectively support clients and families via telephone

Willingness to engage in ongoing education in Hospice Palliative Care

Valid driver's license and access to a reliable vehicle

Clear Vulnerable Sector Check

Preferred:



Certification in Hospice Palliative Care through the Canadian Nurses Association (CHPCN(C)), or willingness to pursue certification.

Experience providing telephone-based nursing care with sensitivity and compassion.

Familiarity with community resources and ability to facilitate referrals as needed.

Other Skills/Abilities:



Deep commitment to the Hospice philosophy of care Demonstrates clinical autonomy and sound judgment. Proficient in Microsoft Office applications (Word, Excel, Outlook). Self-directed with strong organizational and interpersonal skills. Maintains professionalism in all work settings, including in-home and hospice environments. Upholds confidentiality when discussing client information with the interprofessional team.

SALARY:

Range from $ 36.54 commensurate with experience
