About the Opportunity
Reporting to the Early Years Consultant, the Community Home Visitor (CHV) is an essential member of an inter-disciplinary, collaborative Public Health team responsible for the delivery of programs and services according to NS Public Health Standards and Protocols.
The CHV works with families during the prenatal, postpartum and early childhood period and focuses on families who are potentially vulnerable or at risk of poor child health and development outcomes. The CHV uses a family-centered, strengths-based approach to deliver the Enhanced Home Visiting (EHV) Program. Growing Great Kids (GGK) is the foundation for the home visits.
The CHV works collaboratively with the Early Years team (Public Health Nurses, EHV Coordinator, Administrative Support, Early Years Consultant and Manager) to ensure quality case management and service delivery for the Enhanced Home Visiting program.
