POSITION SUMMARY





Reporting directly to the Health & Wellness Manager, the Community Home & Health Nurse is responsible for assisting in the implementation and evaluation of the home & community care, Adult In-Home Care, Diabetes, Chronic Disease Program, and Community Health Program.

The Community Home & Health Nurse delivers and assists with supporting clients in their homes during times of acute or chronic health issues. The Community Home & Health Nurse provides nursing care to clients of the program such as catheter insertion, wound care, foot care, and nursing assessment. The Community Home & Health Nurse works with family members and community resources


EDUCATION/EXPERIENCE




RN Required Diploma of Nursing from a recognized University Current Registration with CRNBC Foot and Wound Care experience or recent course completion

Valid Driver's License with Acceptable Driver's Abstract Acceptable Criminal Record Check with Vulnerable Sector Search Maintains a high level of confidentiality and uses discretion on all matters relating to the affairs of the Westbank First Nation.

JOB DUTIES/RESPONSIBILTIES




Assist with coordination of the administration of the Home & Community Care, Adult IN Home Care, Diabetes, and Chronic Disease programs Stocks the wound care dressing trolley and home care bag supplies and maintains inventory. Contributes to ensuring a culturally appropriate lens is applied to programs and activities Contributes to ensuring a culturally appropriate lens is applied to programs and activities Assists in maintaining the confidentiality of all interactions and records Provides health support and basic medical services to individuals, families, and groups in the community as directed Supports Home Support Workers and Homemakers (includes education from team) as requested by Home Support Supervisor. Presents and teaches classes in the school, daycare, and community regarding specific health issues and makes referrals to specialized health service providers and other professionals as required Assesses for health conditions such as diabetes, heart disease, and tuberculosis using appropriate screening devices Provides health counseling and support to individuals, families, and groups in the community by: + Answering phone calls and inquiries regarding health programming + Responding to individual requests for health information
+ Making post-natal home visits and providing breastfeeding information to support new families
+ Ensuring appropriate referrals to health professionals
Provides community education, support, and activities by: + Delivering group education related to health issues through a variety of approaches including written resources

+ Teaching appropriate puberty and healthy sexuality to children, youth, and adults
+ Working with diabetes program to coordinate teaching sessions in the school and community
Provides specialized nursing services by: + Participating in interagency working committees, workshops, and education sessions
+ Contacting and making referrals to Community Health agencies as appropriate
Have a flexible schedule with availability to work evenings and weekends Performs all duties and responsibilities in accordance with Westbank First Nation policies, standards, and procedures and as directed by the Health & Wellness Manager Maintains a high level of confidentiality and uses discretion on all matters relating to the affairs of Westbank First Nation Maintain standards of conduct as per Nursing Standards and LPN Association Performs all duties and responsibilities in accordance with the Westbank First Nation policies, standards, and procedures

The incumbent must have proficient knowledge in the following areas: + Program planning and evaluation + Up-to-date clinical practices
+ Childhood and Adult health
+ Chronic diseases
+ Case management and confidential record keeping
+ Effective teaching methods
+ Nursing standards and applicable legislation & regulation
+ Assessment tools for a variety of ages and conditions
+ General health and disease prevention
+ Cultural awareness and sensitivity
+ Prenatal and postnatal physiology, nutrition, breastfeeding, labour, and delivery
+ Teaching methods for all ages

Supervision of program staff Strict confidentiality in performing the duties of the Relief Community Care Nurse
