Position Overview:
Under the strategic direction of the Manager, Health Services, the Community Health Worker functions interdependently with staff to provide shared, client-centered care that emphasizes health promotion, disease prevention and client education. The Community Health Worker collaborates with primary and allied care, community services and agencies to facilitate the connection with our clients.
In this role, you would contribute to an integrated team-based model focused on creating impactful care pathways related to social services. You will be pivotal in fostering an inclusive community health setting where equity-deserving and people with physical disabilities experience the highest quality access to care. Summary of Duties:
Develop and roll out various outreach strategies in the community linked to identified needs.
Develop and maintain both informal and formal community networks and partnerships and work effectively with community partners to implement collaborative programs and initiatives, and support capacity building.
Assist the Health Promotion Team in the promotion of programs, services and special events.
Support individuals with developing care plans in relation to the social determinants of health.
Plan, facilitate and implement information sessions, workshops, and group programs related to social services that meet the needs of our populations (people with physical disabilities, seniors, people with lived experience of homelessness and substance use).
Refer community members internally and externally as appropriate.
Identify, compile and maintain lists of culturally and linguistically appropriate community services, programs and resources for use by clients, program participants, community members and staff.
Assist in program data collection, monitoring and evaluation.
Submit program statistics and reports on a timely basis. Manage all expenditures and reporting as per organizational policies.
Assist in other duties as assigned that are reasonable within the scope of the job.
Qualifications/Skills:
Diploma or degree in Social Work, Community Services or a related health/human services field.
2-3 years in a community-based setting working with vulnerable populations.
Demonstrated ability, commitment to and knowledge of the community health care model including the social determinants of health and harm reduction principles.
Knowledge of available social services resources within the City of Toronto.
Proficiency in the use of computers and various software applications, including experience using Electronic Health Record Systems (EHRs)
Experience using PS Suite is an asset
Experience with documentation related to case management and data collection.
Demonstrated ability to work independently and as part of an interprofessional team
