The Community Health Worker (CHW) plays a key role in the Community Care & Navigation Department, supporting the Wabano Mobile Primary Care Outreach Van, assisting with clinic intakes, and providing system navigation and advocacy for clients. This is a front-line, community-based position that helps ensure everyone--especially those facing barriers to care--can access the support they need.


Responsibilities

Reporting to the Team Lead for Primary Care Outreach, the CHW:


Drives and coordinates the Wabano Mobile Primary Care Outreach Van, ensuring it is stocked, safe, and ready for community service. Provides on-site outreach support including client registration, coordination with health staff, and ensuring a welcoming environment. Assists with intake and clinic support, helping clients complete forms, navigate referrals, and record information accurately in the EMR. Offers system navigation and advocacy, helping clients connect with health, housing, mental health, and social services. Delivers health promotion and community engagement activities, including education, outreach events, and cultural programming. Maintains accurate, confidential client records and contributes to program evaluation and reporting.

Required Qualifications

Indigenous candidates who meet the requirements will be given priority. Postsecondary education in Community Health, Social Services, Indigenous Studies, or a related field--or an equivalent combination of education and experience. Minimum 2 years' experience in community health, outreach, or social services. Valid G Class Driver's License, clean driving record, and over 25 years of age (required for company insurance coverage). Knowledge of Indigenous cultures, traditions, and social determinants of health. Experience working with Indigenous communities and/or marginalized populations. Strong interpersonal and communication skills with the ability to build trust and rapport. Proficient in basic computer programs and data entry (EMR experience an asset). Demonstrated ability to work independently and collaboratively as part of a multidisciplinary team. Able to lift and organize supplies and equipment for outreach activities. Willingness to be vaccinated and follow infection prevention protocols.
