Position Summary
: We are seeking a Registered Nurse dedicated to client-centered, family focused centre, to support our Home Care and Community Nursing positions. The Community Health/Home Care Nurse combines her/his health sciences knowledge with health promotion teaching, assessment, clinical nursing skills to effectively assess and coordinate client services.
Qualifications:
Member in good standing of the College of Registered Nurses of British Columbia (CRNBC) with current practicing registration as an RN with the BC College of Nurses
Baccalaureate degree in nursing from a Canadian University
Preferred experience in public health or First Nations Community Nursing
5 Years recent related clinical nursing experience
Valid Class 5 Driver's License and own vehicle with business insurance
Current CPR certification
Updated Criminal Record Check
Knowledge, Skills, and Abilities:
Knowledge and understanding of First Nations communities, health and social issues
Assess patients to identify appropriate nursing interventions
Administer medications and treatments as prescribed by a physician or according to established policies and protocols
Maintain accurate client files with all clients' medical history and consent forms updated yearly
May teach and counsel patients and their families/supports on health-related issues in collaboration with allied health professionals and with Kitsumkalum Health Staff
Knowledge and understanding of the population health approach and the social determinants of health
Physical ability to preforms the duties of this position
Manage time and resources, implementing activities and collaboration across disciplines and related activities
Excellent written and interpersonal communication skills and the ability to work effectively with a variety of people and circumstances. To work with Kitsumkalum Health Staff and Allied Health Professionals
Ability to teach and facilitate learning while incorporating several teaching styles to deliver health promotion activities and services
Utilization of Community Development skills to plan, organize, deliver and evaluate health promotion activities and services
Self-directed with demonstrated organizational skills
Deliver immunization clinic to our members, for babies, youth, and adult
Tolerance and sensitivity to the needs of every individual and their right to be treated with respect and fairness
Appropriate verbal and non-verbal communication patterns, behaviours, skills and knowledge
Ability to maintain a professional demeanour and confidentiality
Ability to incorporate knowledge of Social Determinants of Health
Ability to complete initial and ongoing client assessments, provide nursing care through appropriate prescribed technical therapeutic safety type of interventions
Ability to communicate effectively with the clients, families, the public and members of the interdisciplinary team using verbal, written and computer communication
Ability to create culturally safe, trauma informed health care settings through individualized care planning and collaborative approaches to service provisions, including indigenous health service
Responsibilities:
Participates in the planning, development and evaluation of the Kitsumkalum Community Health Program including health promotion, Chronic Illness Management, Maternal/Child Health, Communicable Disease Control, Canadian Prenatal Nutrition Program and Adolescent Health
Participates in Community events, Daily, weekly and monthly home visit, Engage with our School within the community of Kitsumkalum for Health Teachings on awareness campaign and health promotion
Coordinate services in such a way to meet the community care service needs of the clients focusing on a holistic approach to health to support elderly members health care needs, people living with disabilities, chronic illnesses and wound management
Capability to promote person-focused care that demonstrates empathy for patients and families/care givers, sensitivity to diverse cultures and preferences, patient advocacy and social justice concerns.
Works in collaboration with the Health Director when referrals to assess client need for Community Health/Home Care Services. In collaboration with the Health Director completes a structured client care plans and assessment that includes ongoing re-assessment and determines client need and service allocation and to work with
Provides in-home and office based Maternal/Child programs (prenatal teaching, postnatal assessments, breastfeeding support, group programs, Well Baby Clinics and other programs and services as required
Provide home visit's, office visits, and hospital visits
Experience working with Electronic Medical Records
Provides office based Communicable Disease Control programs and services (immunizations, TB screening and follow up, pandemic plans and other programs and services as required
Plans, organizes, delivers and evaluates Health Promotion activities (Diabetes, Hypertension, Obesity, physical activity, nutrition, sexual health, Harm reduction, Screening champions and Cancer etc.)
Documents the assessments, care plans and service delivery on the clients personal Health Record
Completes and submits service delivery data and reports as required by FNHA on Community Health Program and Kitsumkalum Health Services (Monthly, Quarterly and Annual Reports)
Adheres to strict infection-control guidelines set out by BC CDC standards
Maintains client confidentiality and ensures that all records are stored securely locked up
Provides clients and family caregivers with health information as required through demonstrations and informal teaching sessions
Advocate for client's health care needs
Initiates, facilitates and participates in case management as appropriate
Maintains nursing skills through attendance at workshops, conference, in-service and staff meeting, in addition to reading current literature and Best Practice Guidelines
Maintains professional support and consultation to ensure the program is delivered in a safe effective manner
Establishes internal and external connections with other professionals and community resources

Pay: From $55.00 per hour
