Overview


The Community Health Nurse is responsible for developing and implementing a comprehensive Community Health Nursing Program for residents of Naicatchewenin First Nation, while also providing health education and registered nursing care to community members through proper health clinical programming and home visits. This is a full-time position working Monday to Thursday 9:00AM - 4:30PM and Friday 9:00AM - 2:30PM.

Duties & Responsibilities:



Identifies present and potential needs and resources related to the health of individuals, families and the community; Shares in identifying present and potential needs and resources related to First Nations health services and nursing responsibilities; Plans a comprehensive public health nursing service to individuals and families in their home: Participates in planning for public health nursing in special settings such as schools, place of employment, daycare and other community-based institutions, clinics and health workshops; Contributes to planning for the development and operation of the Health Program; Participates in planning with other agencies for community health programs by advising or participating in community and tribal group planning relating to nursing and health services; Helps provide comprehensive public health nursing services to individual and families in their home; Provides public health services in special settings; Participates in the development and operation of the Health Program; Participates in community health programs; Evaluates the effectiveness of the First Nation Community Health Nursing Program; Uses available help to study and evaluate own job performance and plan for continuing professional growth; Engages in surveys, studies and research; Conducts disease screening and deliver immunization programs; Assess patients to identify appropriate nursing interventions; Administer medications and treatments as prescribed by a physician or according to established policies and protocols; Monitor, assess, address, document and report symptoms and changes in patients' conditions; Operate or monitor medical apparatus or equipment; Responsible for assisting with the recruitment, orientation, and training of Health Services team members; Provides ongoing coaching and development of staff members through goal setting, performance management and regular feedback; Ensures effective implementation and adherence of all professional and organizational standards, policies and procedures; Works collaboratively with the director of Health and Social Services to set goals and objectives for the health team to work towards; Works collaboratively with the director of Health and social Services to assist with reporting requirements; Builds cooperative working relationships with community members, partnering organizations, and other service providers; Maintains and promotes safety by being a proactive safety leader and by following safe work practices and policies as prescribed; Performs other related duties as assigned.

Qualifications:



Must be a Registered Nurse who is currently licensed in the Province of Ontario with no restrictions; Current registration of RNAO or with LAP coverage; 3-5 years' experience working as a Registered Nurse in Ontario; 3-5 years of supervisory / management experience will be considered an asset; Must possess valid driver's license, have clean driver's abstract, and be insurable; Must possess or be willing to obtain current First Aid and CPR qualifications; Ability to communicate in Anishinaabemowin an asset; Excellent communication and presentation skills; Ability to work independently and be a pro-active team member; Must have ability to listen and communicate in a positive manner; Must have computer proficiency and ability to learn new software; Must have the ability to communicate effectively in both oral and written form; Must have demonstrated ability to work with First Nations as well as health and social service agencies and understanding cultural norms, protocols, and diversities; Committed to ongoing learning and development; Employment is conditional upon successful completion of a vulnerable sector criminal records check. Employment is conditional upon successful completion of a drug and alcohol check.
