The Community Health Manager role will report directly to the President & CEO, of the Just Like Family Home Care Victoria & Saanich Peninsula on Vancouver Island and will be responsible for the overall coordination of all aspects of the Just Like Family Home Care business to ensure the efficient day to day running of the company. Development, support, and feedback will be provided from a number of professionals on an ongoing basis including being part of a network of leaders with similar roles from regions across Canada Community Health manager (CHM) Key Responsibilities

  • Building relationships with potential referral partners (to include but not be limited to social workers, hospital care staff and care facilities) throughout our region by telling the story of Just Like Family, telling the story of our clients\xe2\x80\x99 experience, and telling the story of the caregivers\xe2\x80\x99 experience.
  • Supporting Care Relationship Leads (Schedulers) with the hiring, engagement, and development of approximately 45 Caregivers
  • Keeping accurate records with attention to detail
  • Advocating for both the caregivers and the clients. This is like being a matchmaker and paying attention to the multiple stakeholders.
  • Developing client care plans with new clients and their respective family members as is appropriate. There will also be a need to follow up to ensure that that these plans happen while at the same time having deep care for the client which allows for unique personalized care plans (Deep care means measuring the follow through and impacts of our care.)

Supporting team members to care deeply and bring their best selves to this important work every day. \xc2\xb7 Office leadership including ensuring that there is vacation and sickness coverage. \xc2\xb7 Financial & payroll reporting and analysis eg: accurate client invoicing and bi-monthly payroll. \xc2\xb7 Ensuring the compliance and structured implementation of the various people policies and practices The Community Health Manager will also work closely with the President & CEO and the Just Like Family National Marketing team to increase the company\'s revenue through sales and marketing activities in the healthcare field (including exploring and pursuing new referral sources, to maintain and solidify relationships with existing sources, social workers, people in hospitals and care facilities) and other community activities (networking events, meetings, community events). Job Type: Full-time 37.5 hours, Permanent The Community Health Manager position will be a full-time position, reporting to our office at the Cook Street Village Activity Centre (380 Cook Street, Victoria), Victoria, Monday through Friday.
