The Community Engagement / Independent Living Supports Navigator plays a vital role in fostering inclusion and supporting independence for persons with disabilities. This dual-role position is responsible for identifying and promoting accessible community opportunities while also providing support to individuals engaged in the Independent Living Supports (ILS) program.
Key Responsibilities
1. Community Engagement and Inclusion
Research and identify inclusive and accessible events, programs, and activities across the community that align with the interests and goals of persons with disabilities.
Build and maintain partnerships with local organizations, businesses, and service providers to advocate for and expand inclusive opportunities.
Create and maintain a calendar of community-based events and activities, ensuring accessibility and relevance to ILNS participants.
Encourage and support community participation by facilitating introductions, supporting attendance, and addressing potential barriers (e.g., transportation, communication needs).
Promote awareness of inclusive practices and disability rights among community partners.
2. Independent Living Supports Program Support
Provide assistance with the day-to-day delivery of the ILS program, including client support, documentation, and communication.
Support individuals in setting and working toward personal goals related to independence, community living, and social participation.
Assist in the coordination of services, resources, and supports based on individual needs and preferences.
Document client interactions and provide regular updates to the ILS team.
Maintain confidentiality and uphold person-centered, trauma-informed, and inclusive practices.
