Community Disability Support Practitioner ("CDSP")

A day in the life of the CDSP:



Collaborate with other caregiver professional within the Individuals support network to assist in treatment and support plans. Foster trusting, positive relationships to ensure Individuals feel secure and respected. Positively influence Individual's community inclusion and social interactions and engage in activities and encourage self-advocacy. Assist Individuals to set goals, create action plans to reach their full potential and encourage independence. Provide support and assistance to Individuals with disabilities in their daily activities Assist with personal care tasks, such as bathing, dressing, and grooming Help Individuals with meal preparation and medication management Support Individuals in participating in community activities and social events Assist with transportation to appointments and outings Document and maintain accurate records of services provided Collaborate with a team of professionals to develop and implement individualized support plans

Requirements & Qualifications



Previous experience working with adults with developmental disabilities. Ability to provide a satisfactory Criminal Record Check with a Vulnerable Sectors Search. Enhanced ability to de-escalate and/or respond proactively and appropriately to behaviors of concern. Experience with mental health concerns a major asset Effective verbal and written communication and the ability communicate clearly with various clients, colleagues, and Agency Leadership is vital for performing successfully in the role. Intermediate computer skills

Job Type:

Full time and Part time Permanent

Pay:

$22.00-$24.04 per hour

Schedule:

We are looking for candidates available to work

Monday to Friday, between 9:00 AM and 5:00 PM

. Both

full-time

and

part-time

positions are available.
