Waypoint Centre for Mental Health Care is a 315-bed academic and teaching hospital that provides specialized mental health, addiction and geriatric care. Located on the shores of Georgian Bay, we serve some of the most complex and disadvantaged individuals in Ontario and are the sole provider of high-secure forensic mental health services in the province.


The Manager of Outpatient Services is currently seeking a Full-Time Community Clinician - Registered Nurse. The purpose of the position is to provide nursing and mental health services to individuals with a serious mental illness living in a community setting and/or preparing to live in the community, using intensive/assertive community treatment and psychosocial approaches to reintegrate into the community and nursing expertise and leadership within the multidisciplinary team.

KEY ACCOUNTABILITIES:

Assess and record client treatment and responses, assessing activities of daily living and general health issues through interviewing and observing clients, family, significant others and other service providers, administering clinical/ risk assessment tools (e.g. mini mental status) and by reviewing medical and other relevant documentation; and assessment and intervention for metabolic syndrome. Work in collaboration with multidisciplinary team and community partners to provide, monitor, assess, teach, and negotiate and coordinate aspects of provision of medication. Communicate with all stakeholders regarding medication related issues. Ensure required laboratory testing is completed and communicated as required. Assess, plan, implement/coordinate, evaluate, modify and record therapeutic treatment strategies in collaboration with client, family, significant others, and other service providers to meet client goals through supportive counseling, symptom management and/or group therapy. Support client rehabilitation and community reintegration by assessing rehabilitation/discharge readiness and develop, implement, evaluate and modify, in collaboration with the client, family and other service providers, holistic rehabilitation/discharge goals; supporting goals through life skills and health teaching and through developing the clients' relationship with community resources and support. Plan, implement, and evaluate crisis management strategies with client, family and other service providers and provide direct/indirect crises intervention e.g., education for other service providers; providing risk management strategies as required e.g. risk appraisal tools, direct supervision of forensic clients; and to report appropriate observations to supervisor that may affect client care, treatment, or safety; provision of services to support Community Treatment Order legislation; Utilize components of the case management model to provide comprehensive individualized assessment and planning, service coordination of mental health issues, consultation with clients, family, significant others and other service providers; liaison and advocacy; resource development; direct service provision, monitoring and evaluation. Liaison with community agencies, facilities in order to serve as a resource for educational / clinical consultation in the community, to meet the needs of the population being served, their caregivers and to improve public awareness of mental health issues; representing Waypoint/specific program on community committees and in other forums; provide information to community agencies/groups; promote and advocate for community based programs services. Participate in research and program evaluation projects, participate in hospital and program committees; engage in ongoing education; document all outpatient contacts for statistical purposes as per hospital/program policy. Provide clinical leadership within the multidisciplinary teams. Leadership is demonstrated through processes of orientation and professional mentoring and supervision of nursing students and non-regulated staff. Engage in Reflective Practice and continuous learning designed to improve the application of knowledge, skill and judgment in the provision of client-centered care.

REQUIREMENTS/QUALIFICATIONS:

Current registration as a Registered Nurse with the College of Nurses of Ontario is required. Valid certification in Basic Life Support (BLS) with Oxygen Therapy (OT) and Airway Management (AM), along with a valid Ontario Driver's License is required for the role. Knowledge of psychiatric diagnosis and disorders, theories of general and mental health nursing and practice that is normally gained through advanced preparation at a baccalaureate level from an undergraduate nursing program or equivalent experience, to provide therapeutic nursing intervention and /or advisory services through the development, implementation and modification of treatment plans which aim to ensure the best level of care for the client being treated in the community and/or community facilities or admitted into a hospital. Demonstrated knowledge of best practices and relevant research in an area of specialty. Demonstrated knowledge of relevant sections of relevant legislation - e.g. Mental Health Act and Community Treatment Order protocols, Long Term Care Act, Health Care Consent Act, Criminal Code of Canada, Personal Health Information Protection Act, Homes for Special Care Act, Regulated Health Professions Act...etc - to ensure compliance and consistency, and to ensure that legislated client entitlements such as consent to treatment are observed. Knowledge of specific criteria and requirements related to Community Treatment Orders. Proven experience in counselling techniques (e.g. supportive), life skills and health teaching techniques and symptom management in order to support the development, implementation, evaluation and modification of client goals and activities. Experience with psychotropic and other medications and their side effects in order to assess client reaction by monitoring the use and effect of medication and administer as required. Ability to perform Cardiopulmonary Resuscitation (CPR) and First Aid in order to respond to a medical emergency. Specific knowledge of community services and agencies that serve the client in order to assist them in achieving their goals by linking them to those services. Techniques of data acquisition in order to assist with research and program evaluation projects. Effective use of risk management policies/tools in order to assess risk management activities (e.g. administration of risk management tools, direct supervision of forensic clients) and to ensure client and personal safety when practicing in a community setting. Working knowledge of computer software packages to produce relevant documentation using word processing, e-mail, calendar & inter/intranet. Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results. Demonstrated leadership, conflict resolution, interpersonal and organizational skills. Proven written and verbal communication skills. Proficiency in English and French language skills is preferred.
