Whether their disability is physical, the result of brain injury, or aging, our more than 2,000 team members assist clients in their homes and across communities, to live as independently as possible.
The opportunity:
We are currently looking to fill One Full-Time Community Case Coordinator role in our Durham region - Office located in Whitby, ON - 35 hours weekly.
Location:
1615 Dundas St E, Whitby, ON L1N 2L1
Schedule:
Monday to Friday with oncall rotation
Responsibilities:
Community Case Coordinator works within a team supporting an Outreach Attendant Services Program, working closely with and at the direction of the Supervisors and Program Manager. Services provided to individuals with physical disabilities include personal care and activities of daily living to support clients to remain independent in their home environment
Will often work with community partners to ensure the best outcome for our clients
Has a client focused lens working closely with supervisors and with their direction
Works collaboratively to provide client orientation, care planning and ensure that service contracts are accurate and done in a timely manner.
Will complete regular assessments of client needs within their own home environment within a timely manner
Will work closely with the Team Lead to ensure client specific training is completed as clients are onboarded and as new staff are brought onto client's service
Will periodically support scheduling and coordinate staff using internal scheduling system
A team approach to problem solving is required, as well as direct contact with clients, staff, community partners and MODC management team. Building professional relationships with collaborative partners, community support agencies and hospital discharge planners is essential for arranging coordination of services
Perform duties in a professional manner, which respects the dignity and right to privacy and maintains client and staff health and safety
Travel throughout the Durham Region is required to complete assessments, and to support service contract renewals, and to meet with clients related to their services or concerns
Typically scheduled to work regular business hours, however MODC provides after business hours support to clients and the CCC will participate in an on-call rotation
Ensure the Collective Agreement is adhered to
Qualifications and experience:
Degree or Diploma in Human Services/Healthcare education. Preferred background in
Nursing, Social Service Worker or Social Work.
2 - 5 years' experience coordinating services for people with disabilities or seniors highly desirable
Valid driver's license and reliable vehicle are required
Community-based experience working with seniors or individuals with a disability
Highly organized and excellent time management skills
Excellent communication skills, written and verbal in English; Bilingual English/French is considered an asset
Demonstrated ability to train employees including organizing, prioritizing and scheduling work assignments
Experience working in a unionized environment an asset
Problem solving and conflict resolution skills
Strong observation and assessment skills
Knowledge of behavior management principles
Current Standard First Aid and CPR certification
