Job Description

Kilala Lelum Health and Wellness Cooperative is seeking a Registered Nurse (RN) with clinical and clinical education experience to join the Primary Care and Research and Evaluation teams. This position is best suited for a nurse committed to harm reduction, sexual wellness, and health equity, and who is dedicated to delivering compassionate, culturally safe care to Indigenous people who use drugs (IPWUD) and individuals living with HIV and/or HCV.


Kilala Lelum aims to provide access to primary care, STBBI screening, food security, medication management, cultural support, and connections to Elders, Knowledge Holders, peers, and social services to new and existing members of the Health Centre through an outreach-based, culturally safe, trauma-informed, and harm reduction approach.

Job Summary




The

Clinical Resource Nurse

, supported by a multidisciplinary team, will provide excellent, culturally safe care to members accessing the Primary Care clinic, as well as those supported by other Kilala Lelum programs and services, such as Doris Fox Lelum, our Indigenous Wellness and Resource Hub. Working in conjunction with primary care providers (e.g., Physicians, Nurses, Nurse Practitioners), Allied Health providers (e.g., Counsellors, Social Navigators, Dieticians), and the Cultural Wellness team, the Clinical Resource Nurse will play a key role in the C-CARE Project, an initiative led by Kilala Lelum, in partnership with the BC Centre for Disease Control (BCCDC).


The project aims to identify, recall, and link members with untreated or unscreened hepatitis C infection to care and treatment, supporting BC's goal of eliminating hepatitis C by 2030. The Clinical Resource Nurse will provide clinical leadership, direct member care, staff mentorship, and quality improvement expertise to ensure the successful implementation of culturally safe HCV screening, testing, treatment, and follow-up within Kilala Lelum's clinic and outreach services.

Key Responsibilities:



Clinical Care & Leadership



Coordinate and provide wraparound, trauma-informed, culturally safe and humble HCV care, including HCV screening, confirmatory testing, treatment initiation, and sustained virological response (SVR) follow-up. Works alongside a multidisciplinary team to support members facing barriers, including but not limited to unstable housing, lack of phone access, and stigma. Act as a liaison between members, Elders and Knowledge Holders, other service providers, and community partners.

Education & Mentorship



Provide clinical expertise and mentorship to staff (e.g., nurses, social navigators, outreach workers, physicians, community members) regarding HCV and care workflows. Deliver staff training, micro-trainings, and peer mentorship on registry use, linkage-to-treatment processes, and culturally safe, trauma-informed practices. Promote member education and support around self-care, wellness, and treatment options.

Quality Improvement & Research



Participate in Continuous Quality Improvement (CQI) activities: monitor outcomes, track care cascade indicators (e.g., screening, treatment initiation, completion, and SVR). Contribute to evaluation activities, including data collection, chart audits, safety reviews, surveys, and Sharing Circles with members. Support knowledge translation through reports, presentations, and toolkits to inform scale-up across BC.

Community & Systems Integration



Collaborate with BCCDC, provincial partners, Elders, and Knowledge Holders to co-develop strategies for reconnection and recall to care, outreach materials, and culturally safe, trauma-informed messaging. Engage in community outreach through mobile services, drop-in events, and peer navigation supports.

Qualifications & Skills:



Required:



Current BCCNM registration (RN) and CPR certification. 2+ years of primary care/harm reduction/community health nursing (preferably in the DTES). Demonstrated experience in hepatitis C, STBBI care, harm reduction, and substance use health. Strong knowledge of primary care nursing, chronic disease management, and the HCV cascade of care. Skills in quality improvement methods, data-informed practice, and EMR/registry systems. Excellent communication, teamwork, and interdisciplinary collaboration skills. Strong case management, advocacy, and crisis intervention skills. Knowledge of trauma-informed, Indigenous cultural safety and humility frameworks. Ability to lead teams, coordinate workflows, and problem-solve collaboratively. Knowledge and understanding of STBBI screening and treatment guidelines.

Preferred:



Certified Practice in OUD and STBBI/Reproductive Health. Experience training others in naloxone administration and overdose response. Experience in nursing-related education, quality improvement, and leadership roles. Familiarity with Truth and Reconciliation Calls to Action and BC Declaration on the Rights of Indigenous Peoples Act.

Working Conditions:



Physical ability to carry out the duties of the position. Work will take place in the primary care clinic at 626 Powell Street with occasional outreach in the DTES.

Supervision Provided:




This position will receive support from the Research and Evaluation Lead, the Primary Care Co-Leads, the Medical Lead, the HR & Admin Lead, and the Executive Director.

Additional Information:



This is a non-union nursing position with an hourly salary based on the BCNU wage grid. The ideal working days are to be determined by the successful candidate to meet the requirements of 0.6 FTE (24 hours per week). Successful candidates will be required to complete a Criminal Record Check. Robust extended health and dental benefits are available.
