Summary Related to My Work:
I am ultimately responsible to the tenants. I work to defeat bureaucracy and ensure that no decision is made without asking the vital question... "What do the tenants say?" This is wise leadership. A wise leader also utilizes the knowledge of the tenants and care partners to help build a place where people want to live and work.
The coordinator models the person-directed care and takes an active role in "Edenizing", building neighbourhoods and showing the greater community around us that life for the tenants in our home is a life worth living.
Required Qualifications:
1. I am a professional nurse registered in good standing with my college of nursing in Manitoba and have at least three years of leadership experience in a health care setting to effectively oversee staff and all activities and services within the health care setting. Senior care or long-term care experience is preferred.
2. I have a practical working knowledge of computers, devices, and associated programs/apps.
3. I communicate with well-developed written and verbal communication skills. I can fluently read, write, and speak English. My ability to speak a second language is considered an asset.
4. I demonstrate empathy, concern, and interest in providing quality care and service to our tenants.
5. I interrelate well with others and establish positive relationships with tenants, their families, other staff members, volunteers, and the public.
6. I can adapt quickly in a progressive change-oriented environment.
7. I work independently and can evaluate my own progress and take responsibility to grow in
my position.
8. I am interested in learning more about person-directed care and how it applies to work within our community of seniors.
9. I can manage the associated physical, emotional, and psychological demands of the work involved.
My Responsibilities in a Person-Directed Care Environment
1. I review and apply the principles of person-directed care to my work everyday.
2. I participate in creating an interesting human habitat/neighbourhood for the tenants.
3. I recognize the plagues of loneliness, helplessness and boredom in the tenants and work to alleviate them whenever possible.
4. I plan for and provide options that take the tenant's opinions and choices into account.
5. I look for ways for my tenants to have continuing contact with animals, children, plants, and myself.
6. I host neighbourhood meetings to hear what the tenants have to say and follow through to address concerns brought to my attention.
7. I work to build others up with positive feedback so that they can grow.
My Responsibilities Related to Safety and Managing Risk
1. I operate an effective, efficient organization involved in Risk Management and Continuous Quality Improvement to provide the highest possible standard of care to the tenants.
2. I must perform all work in a safe, budget conscious and efficient manner. I will maintain the building and work area in a clean, orderly, and safe condition keeping waste to a minimum.
3. I am required to follow manufacturer's instructions and safe work procedures in the operation and use of all equipment and/or chemicals.
4. I will maintain a satisfactory level of health and physical fitness so that injury free and regular attendance at work can be maintained.
5. I perform all food related activities in accordance with prescribed sanitation and food handling techniques.
6. I am required to review, understand, and practice all disaster codes, prevention, and related safety measures within the building a minimum of monthly.
7. I review, understand, and practice infection control measures including "routine precautions".
8. I adhere to all related policies, the Mission Vision and Values, PHIA, PPICA, our Ethics Framework and the Citizen Bill of Rights for Bergen Gardens.
9. I report all tenant and staff safety concerns to the Community Care Director immediately.
10. I will attend relevant education sessions and conferences as required to ensure that my skills and knowledge are developed and upgraded so that I can work safely.
11. I must provide teaching, mentoring and feedback to all new and visiting care partners.
12. I accurately document and submit all occurrences that have taken place.

Related to my Clinical Resource Nurse (CRN) Responsibilities:
I oversee the quality of the health care services for the tenants at Bergen Gardens including:
1. I supervise all nursing best practice operations for the building including infection control, education,
training and policy and procedures, to ensure that the highest nursing standards are upheld for Bergen Gardens.
2. I am an active member of the team that recruits, interviews, hires, disciplines and schedules suitable employees for the entire nursing department and I lead those staff entrusted to me with confidence.
3. I participate as a member of the Community Response Team when on site managing emergency calls for the building and act as a consultant for the nursing staff as needed when on call after hours.
4. I consult with the Community Care Director to review and advise on all tenant applications for the Cottages and their need for nursing services to ensure that their care needs can be met at Bergen Gardens.
5. I collaborate with families and tenants to set up Self Family Managed Care (SFMC) for tenants as requested and manage all staff on my team to deliver exemplary nursing services to tenants as a part of SFMC.
6. I assume responsibility for the nursing care in the absence of the Community Care Director.
7. I participate in community visits, interdisciplinary team conferences, committees and meetings as requested.
8. I report monthly to the Community Care Director with respect to my program activities, attendance statistics, action plans, statistical analysis, and operational issues and/or concerns as a part of the CQI program.
9. I participate in and monitor the budgeting process for my work area in conjunction with the Community Care Director to manage operations within the determined budget especially related to staffing costs.
10. I make recommendations to the Community Care Director for additional equipment, trades, or other capital expenditures outside of budget and on approval, arranges for purchase for the Cottages.
11. I participate in the afterhours call clinical support for nursing staff in the building on a rotation.
