Summary

Within the context of a value-based patient, client and family centred model of care and, in accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, promotes a safe, respectful and civil working environment for patients, residents, families, visitors and staff.

Reports to the Patient Care Manager (PCM), the Clinical Program Coordinator (CPC) provides leadership in the development and implementation of optimal program processes and procedures to ensure that all patients with inherited blood disorders, in the province of BC are appropriately linked to the Adult Inherited Blood Disorders Program of BC/Yukon (PAIBDP). Determines patient priority and access needs, and works with clerical staff to ensure optimal scheduling is met for all patients. This includes appointments for regular or episodic clinic care, in-person at the Vancouver site, outreach or via telehealth (zoom/phone) as appropriate. Provides regular data reports to the team, facilitates capture of patient reported or provider reported clinical outcomes using the most appropriate IT interface (i.e. Thrive Health) and helps prepare summaries as needed.

The CPC works with the interdisciplinary care team to implement and evaluate changes to increase efficiency of, and access to, high quality patient services in order to deliver health outcomes that matter to patients. Determines priority access needs and optimal scheduling requirements for patients whether for regular or episodic care. In collaboration with the PCM, works with the interdisciplinary care team to share resources across programs that optimize the opportunity for patients to access care based on their needs. Works with team on ongoing program development around case management, education, tracking processes and outcome measures.

The CPC works with the PCM to ensure the provincial program services comply with clinical, corporate, provincial and federal standards, processes and reporting requirements. Develops and/or implements ongoing evaluation and quality improvement initiatives to ensure best practices and outcomes.

The CPC works to enhance partnerships, and maintain clear communication with, key stakeholders at all levels both internal and external to the organization. Key stakeholders include patients, families, health care providers, vendors, contractors and equipment providers. On behalf of the PCM, partners with Supply Chain for equipment, product and supply services.

Education

Master's Degree in a clinical health sciences discipline and a minimum of three (3) to five (5) years' recent, related clinical, project management, and/or quality improvement experience in a large, complex health care environment, or an equivalent combination of education, training and experience.

Duties * Assumes a leadership role in collaboration with the interdisciplinary care team in prioritizing and scheduling appropriate and timely patient access to the PAIBDP clinics, outreach and related services.
  • Provides case management for patient. Examples include for the home infusion program when transitioning from hospital to community, the transition of patients onto new and innovative treatments and other situations where case management would benefit patient care.
  • Develops, implements and updates processes/criteria that make the program more effective and efficient in providing quality services to patients. Facilitates the implementation of care processes through coordination of team improvement projects and achievement of related targets/times and objectives. Identifies, leads and supports quality improvement initiatives.
  • Organizes and schedules services at appropriate pre-specified intervals, such as clinics and outreach or telehealth, based on individual patient and program needs. Ensures all in-scope patients in the province are linked to the clinic and are registered in the applicable electronic health record and scheduled appropriately for available services, and ordered investigations are collated within the patients' electronic health records.
  • Collaborates with the clinicians at BC Children's Hospital and the PHC Transition Navigator to ensure that newly transitioned patients are booked at increased frequency to establish connection with the adult program and rapport with providers.
  • Efficiently manages data within applicable electronic health records, online platforms as a data repository and ensures all services provided are captured and reported to the appropriate authority. Ensures optimal use of Cerner, iCHIP, PSLS and other IT systems (i.e. Thrive Health) clinic reports and works with teams to acquire and analyze information to identify service performance and patient outcomes.
  • Works closely with the team on ongoing program development with respect to patient services delivery. Explores options for additional outreach clinics by collaborating with other Health Authorities.
  • Monitors expenditures, identifies financial issues, investigate variances and discrepancies, seeks to adhere to budget and communicates budget status to PCM and PAIBDP team. Maintains accurate and current accounting of all accounts connected to the PAIBDP. Manages the Clinical Consultative Service schedule and compensation submissions. Seeks opportunities to reduce costs and prepares rationale and justification for budget request.
  • Maintains up to date listing of the status of contracts currently under negotiation. Tracks contract negotiations and follows up with sponsors, investigators and others as required.   • Leads, monitors, tracks and evaluates program progress and reports to stakeholders the status of the program. Implements and monitors performance metrics to ensure standards. Makes recommendations regarding program changes and rationale for change, seeks consensus and proceeds as needed. Acts as a resource to the team and staff, provides support and maintains timelines.
  • Ensures the care team complies with established clinical and corporate policies, standards and procedures. Works with the team to evaluate effectiveness of processes, identify opportunities and ensure standards are achieved and maintained.
  • On behalf of the PCM, partners with Supply Chain for equipment, product, and supply services and liaises with Supply Chain on contract wording. Ensures supplies are well stocked and available for use. Establishes patient accounts and troubleshoots patient supply order issues. Initiates and follows up on patient supply audits to ensure the appropriate ordering of supplies. Develops and implements an effective patient feedback process to identify and manage patient concerns, input, and questions about the provincial program services, from initial notification to resolve and patient follow up.
  • Ensures communication of pertinent information to external and internal stakeholders and patients/families using a variety of communication media including verbal, written and electronic methods to facilitate decision-making and timely access to care.
  • Leads the implementation and completion of program pamphlets with team input.
  • Represents Providence Health Care (PHC) on both internal and external committees, as needed, and supports the established priorities of the Provincial Program. Maintains awareness of trends and issues within health care, through education, literature reviews and other means. Acts as a positive role model for the care team by integrating components such as clinical practice, consultation, collaboration, leadership, education, and evaluation.
