The Clinical Pharmacy Specialist/ Leader (CPS/L), assignment as Drug Information/Drug use evaluation and Practice, serves in the role of a leader for medication system of the network. This role, based at SMH but also integrates with other sites, is highly linked with the functions of the Pharmacy Department and pharmacy practice, education and operations. In alignment with the general job description for Clinical Pharmacy Specialist/ Leader, the individual fulfilling this position will:
Provide leadership in network-wide appropriate usage of medications on a macro level
Act as lead/secretary of the network-wide medication related committee(s), in conjunction with other relevant CPS/L
Collaborate with other relevant CPS/L to maintain Drug Information service for hospital staff and physicians as well as to maintain up-to-date resources
Lead in maintenance of network-wide drug formulary and other associated databases (e.g. IV pump libraries
To proactively lead the development, revision, implementation, and evaluation of medication related standards, protocols, guidelines, procedures, and medical directives typically with cross-program or hospital-wide focus; and to promote desired outcomes through education, internal publication and monitoring.
Provide leadership and mentorship to clinical pharmacists, pharmacy students and residents when assigned to assist in medication system-related projects or in answering drug information questions
Provide leadership to a group/community of Practice for pharmacists
Participate or lead committees affecting medication systems where assigned
Support departmental or corporate initiatives in teaching and/or research, quality improvement, workload documentation, service planning, and procurement related functions as assigned
Participate in scholarly work such as internal/external publications, drug use evaluations
Precept pharmacy students and residents on evidence-based medicine, drug information and drug use evaluation
Provide clinical coverage where required
QUALIFICATIONS
Advanced degree in Pharmacy (Doctor of Pharmacy preferred) or equivalent in terms of practice experience
Registration as a pharmacist with the Ontario College of Pharmacists (Part A Register)
Minimum of eightyears pharmaceutical care practice experience in a teaching hospital as well as demonstrating progressive degrees of teaching responsibility
Excellent drug information skills
Strong leadership skills, demonstrating progressive degrees of responsibility and the ability to lead projects, people and corporate committees
Demonstrated skills, knowledge and ability in all the following:
Project management at either departmental or higher level
Mentoring and teaching skills development
Problem solving in a rapidly changing environment
Experience in or demonstrated potential for scholarly activity
Excellent inter-personal, change management and facilitation skills and strong customer focus
Excellent ability to be detail-oriented and communicate verbally/in writing to diverse audiences
