SUMMARY OF DUTIES: Responsible for the provision of Clinical Pharmacy services; resolving and preventing drug related problems through medication profile review, patient assessment, Pharmaceutical Care Plan Development and Implementation. Professional activities include provision of drug information to other health care providers including, physicians, nursing, pharmacy personnel and other allied health professionals. Participants on committees, both internally and externally, conducts Drug Utilization Evaluation projects and is involved in presentation of in services to professional staff.
EDUCATIONAL REQUIREMENTS
Completion of Bachelor of Science in Pharmacy, Pharm D, or undergraduate degree recognized by the Pharmacy Examining Board of Canada (PEBC)
Licensed as a Registered Pharmacist with the Ontario College of Pharmacists
Desirable Experience: 2 years previous hospital pharmacy experience or Hospital Pharmacy Residency
Assets and Abilities: Excellent communication, interpersonal and team building skills
Strong team skills with a proven ability to perform duties independently, with a high degree of motivation and minimal supervision
Strong knowledge of Pharmaceutical drugs, therapeutics with ability to apply during provision of practice
High degree of accuracy and attention to details
Demonstrated computer literacy
Innovative and willing to seek new challenges
Ability to work independently
Ability to meet deadlines
Ability to work on several projects simultaneously
Ability to prioritize work load
Demonstrated ability to respond to drug information questions, perform literature searches and evaluate published research
Proven ability to attend work regularly
Demonstrated proficiency as a hospital pharmacist based on OCP, CSHP and NAPRA standards
Salary Range: $48.21 - $64.50 per hour
