The Clinical Pharmacist is responsible to identify, resolve and prevent drug related problems and contribute to optimal client outcomes. The Clinical Pharmacist is responsible for ensuring that pharmacy services meet the needs of the clients and be administered with accepted ethical, professional practices and legal requirements. He/she contributes to medication management at Holland Bloorview to ensure that the utilization of medications is safe, appropriate and cost effective.

This is a temporary part time (0.8 FTE, or 60 hours in a bi-weekly pay period) position expected to be until March 31, 2024, with the opportunity for additional casual hours.

Key Responsibilities:

Clinical

  • Provides clinical pharmacy services to identify, resolve, and prevent drug related problems, to ensure optimal client care is achieved
  • Designs, implements and monitors drug therapy specific to client care needs
  • Participates in interprofessional rounds and family meetings as a member of the client care team
  • In partnership with clients/families and the interprofessional team, provides client and family-integrated care that is goal directed, evidence based and outcome oriented
  • Provides discharge planning and teaching to clients and families
  • Collaborates with members of the interprofessional team and community providers to ensure an integrated approach to service
  • Participates, when possible, in select outpatient clinics for the provision of pharmaceutical care and medication teaching for clients and families
Drug Distribution
  • Partners with pharmacy technicians to provide a safe and effective drug distribution system
Communication
  • Provides drug information and education to health care personnel, clients and families, and students
Education
  • Ensures professional competency through ongoing continuing education
  • Provides in-service education to physicians, nurses and other healthcare professionals
  • Provides orientation and training for new staff members and students
Research and Clinical Trials
  • Participates in pharmacy related and collaborative interprofessional research projects and clinical drug trials
Risk/Quality Management
  • Provides On-Call pharmacist services, on a rotational basis, as required
  • Participates in quality management activities including regular evaluation of pharmacy services
Administration
  • Participates in quality improvement activities and supports the maintenance of a safe and healthy work environment for staff, clients and families
  • Participates in hospital committees as required
Qualifications:

Education/Licensure
  • Graduate of an accredited Faculty/College of Pharmacy (Baccalaureate Degree in Pharmacy or Pharm D.)
  • Licensed in Ontario to practice pharmacy (Part A of Register)
Experience
  • Two years experience in a hospital pharmacy with demonstrated excellence in clinical practice with a paediatric population is preferred
  • Experience in providing exceptional client and family-centred care

  • Light lifting, walking, standing, sitting at a computer terminal
  • Small exposure to chemical pharmaceuticals including chemotherapeutic agents
