Job Description

The Centre for Family Medicine is currently seeking a Pharmacist (0.8 FTE-1.0 FTE) to cover an 12-month contract with a possible extension.



Our mission is "To provide leadership and excellence in comprehensive patient-centred primary care through education, research, innovation and care provision in a collaborative, sustainable, inter-professional environment".



Our team currently consists of twenty-five family physicians, nurse practitioners, nurse educators, registered practical nurses, health educators, social workers, family and marriage therapists, dietitians, clinical pharmacists and specialist physicians. We are associated with optometry and a chiropractor.



The Centre for Family Medicine's academic Family Health Team is comprised of six sites and is an integral part of the University of Waterloo Integrated Health Building as well as the McMaster Waterloo Regional Medical Campus. We work in a fully computerized environment and encourage continued educational experiences.



Position Overview:



The Clinical Pharmacists at the Centre for Family Medicine provide direct clinical care through a collaborative model and provide pharmacy services to support various clinics. This includes, but is not limited to, Anticoagulation Clinic, Memory Clinic, Hypertension Clinic, Affirm Clinic, Diabetes Clinic, Medication Management.

Additionally, clinical pharmacists provide general pharmacy support to the academic primary care team.



Primary Duties and Responsibilities

Managing patients on warfarin through the Anticoagulation Clinic

Providing collaborative pharmacy expertise in our multi-disciplinary Memory Clinic

Providing general pharmacy support to the primary care team such as drug information, medication reviews and hospital discharge medication reconciliation

May also be involved in the supervision of undergraduate pharmacy learners and pharmacy residents


Qualifications and Experience:



Bachelor of Science degree in Pharmacy from an accredited program of study required Licensed as a Part A pharmacist with the Ontario College of Pharmacists required Teaching experience, capability of teaching and guiding staff of various educational backgrounds Experience working in a family health team or primary care setting Clear understanding of current health care needs and a vision for the future of pharmacy practice Demonstrated ability to take initiative, work independently and as a team member in a fast paced, challenging environment Excellent interpersonal, verbal and written communication, team-management and organizational skills Post-graduate Pharmacy degree (e.g. residency, clinical Master's degree or Doctor of Pharmacy) preferred Experience or credentialing in Anticoagulation is an asset (ie: Management of Oral Anticoagulation (MOAT)) Experience working in a Memory Clinic or credentialing in geriatrics is an asset (ie: Certified Geriatric Pharmacist)
