UNIT PROFILE:
The Consultation Liaison Psychiatry Service works at the interface of medicine and psychiatry and provides psychiatric consultation for patients admitted to medical/surgical units across the hospital. We provide service and expert psychiatric consultation for individuals who develop signs and symptoms of psychiatric distress while they are in hospital for medical or surgical reasons, and help support family members of these individuals.
POSITION SUMMARY:
The Clinical Nurse Specialist (CNS) provides advanced, comprehensive, and specialized nursing expertise in delivering evidence-informed, patient-centred care in accordance with the mission, vision and values of SJHH and the Mental Health and Addiction Program (MHAP), the Standards of Practice of the College of Nurses of Ontario and other relevant legislation, and the Canadian Federation of Mental Health Nursing (CFMHN) Standards.
The CNS requires advanced expertise and in-depth nursing knowledge, skills, and clinical reasoning in both complex mental health and/or addiction concerns and co-occurring medical conditions as this role operates at the interface of care for patients between these services.
The CNS facilitates the interface between psychiatric mental health and acute care and includes responsibilities related to the provision of integrated care for patients with complex mental health and/or addiction and medical co-morbid conditions using advanced expertise and in-depth nursing knowledge, skills, and clinical reasoning. The CNS provides comprehensive psychiatric assessments, facilitates conversations around care decisions and disposition pathways with various members of the interdisciplinary team, and develops and evaluates care plans and treatment/therapeutic interventions for patients in the acute care areas at SJHH. The CNS exercises exemplary decision-making skills in complex clinical situations.
As a member of the interdisciplinary team, the CNS acts as a role model and change agent by providing in-depth nursing expertise in the domains of advanced nursing practice, including: clinical, research, leadership, and consultation/collaboration. Furthermore, this position provides a key leadership role in improving the delivery of care, mentoring colleagues and students, contributing and supporting a culture of safety, quality, innovation, empowerment, accountability and continuous learning.
Nurses in the MHAP are committed to the alleviation of stigma and discrimination related to mental health and addiction, as well as reduction of the use of restraint and seclusion in all of our services. The principles of Person-Centred Nursing, recovery and trauma informed care are foundational to this specialized nursing practice.
QUALIFICATIONS:
Current Registration with the College of Nurses of Ontario as a Registered Nurse (General Class) in good standing - required
Masters Degree in Nursing - required
CNA certification in Psychiatric/Mental Health Nursing (CPMHN(c)) or commitment to complete within 5 years of hire as a condition of employment required
Minimum 5 years recent related clinical experience in psychiatric and mental health nursing
Advanced knowledge of the interplay of psychiatric, addiction, and medical or surgical conditions
Previous experience in a consultation liaison role preferred
-Gerontology Nursing certificate from CNA - preferred
University-affiliated clinical faculty appointment preferred, or eligibility to secure appointment
Membership and active involvement in provincial, federal or international professional nursing associations preferred
Knowledge of professional trends, issues, scope and standards of practice, and other relevant legislation
Knowledge of a recovery and trauma-informed approach to care
Demonstrated theoretical and empirical knowledge bases and advanced level of clinical competence in working with patient with mental health and/or addiction disorders
Advanced knowledge of psychopharmacology
Highly developed communication skills both written and oral
Advanced knowledge of psychotherapeutic modalities and behavioural interventions provided at the medical/psychiatric interface
Ability to generate, synthesize and apply empirical evidence and conduct research
Previous experience in designing or facilitating teaching/educational initiatives
Excellent interpersonal and team collaboration skills
Demonstrated ability to act as a resource and consultant to clinicians and interdisciplinary teams
Demonstrated self-directed, critical thinking and problem solving skills
Demonstrated organizational, presentation and leadership skills
Demonstrated commitment to safety and quality
Demonstrated commitment to continuous learning and ongoing professional development
