Clinical Nurse Specilaist - Geriatrics - MEDICINE PROGRAM

Reporting to the Program Director of Medicine, The Clinical Nurse Specialist (CNS) role will led and anchor the Mackenzie Health Seniors Care Strategy across the organization and sites. The CNS will ensure a comprehensive, specialized, and advanced care strategy for seniors is developed and implemented across the spectrum of inpatient and outpatient areas. The successful candidate will practice within the interprofessional and leadership team model and be responsible for leading and ensure integration of best practices in geriatric care and promote excellence in the nursing care of older adults through directing, developing, and implementing patient care, program design, clinical consultation, staff education and research.

Contribute to safe, quality care by

  • In collaboration with the lead for Senior Care at Mackenzie Health, the CNS will be responsible and accountable for leading and actioning the workplan.
  • Developing goals and objectives for the implementation of a senior care, including timeline
  • Facilitating teams to incorporate and adopt effective delivery of competent, compassionate patient centered care
  • Developing program metrics, monitoring, and evaluating patient care processes and outcomes.
  • Ensuring teams have line of site on metrics and how team will be accountable to maintain senior care locally within their clinical area
  • Utilizing nursing knowledge to meet the current and future needs of seniors, families, communities, populations and the profession by sharing current knowledge.
  • facilitates the understanding and application of research.
  • Leads project team, to ensure timelines are met, while providing ongoing feedback to project sponsorship, while removing barriers to successful completion
  • Promoting an atmosphere of open communication that facilitates staff input into decision-making, resolution of conflict, and collegiality among all health team members.
  • Creating an environment that supports and advances professional practice, teamwork and communication through mentoring and role modeling application of "expert" clinical knowledge.
  • Liaising with members of inter-professional team to ensure excellence in geriatric patient care.
  • Role modelling professional and organizational core competencies.
  • Providing leadership in facilitation, decision making, conflict resolution and clinical abilities.
  • Provides consultation support and management recommendations for clinical needs of the patient population in prioritizing care to a constantly changing population with varied clinical conditions and acuity levels.
  • Identifies recurring clinical issues/problems and communicates findings/suggestions for change appropriately
  • Initiates and maintains links with other geriatric leads in Central Region and OH, to advance clinical outcomes for patients within the region.
  • Designs & provides educational and learning opportunities for staff, based on the identified strategies
  • Ensures policies, guidelines, protocols and practice are based on best available evidence.
  • Applies a broad range of theories and relevant research to clinical practice
  • Promotes a positive work environment that supports reflective practice, life-long learning, creativity and innovation in nursing care
What must you have?
  • Master\xe2\x80\x99s Degree in Nursing
  • Current certificate of competence and member in good standing from the College of Nurses of Ontario as an RN or RN (EC).
  • Minimum of five \xe2\x80\x93 eight years leadership experience
  • Demonstrated current expertise and experience in gerontological nursing.
  • Specialty certifications related to the area of expertise
  • Membership in the Registered Nurses\xe2\x80\x99 Association of Ontario

  • Computer literacy in word processing

  • Ensures best practice standards of care and strong interprofessional team functioning in collaboration with the care team, partnering physicians, Program Director
  • Evidence of program design and resource development skills
  • Self-directed, works independently and able to effectively lead project teams to achieve outcomes
  • Effective communication skills, engages others, and is a reflective listener and learner
  • Demonstrated knowledge of evaluation methodologies
  • Proven analytical thinking and proactive problem solving and decision making skills
  • Excellent report writing skills
  • Proficient with computer programs including; Word, PowerPoint, & Outlook.
  • Working knowledge of the College of Nurses of Ontario standards of practice.
  • Proven attendance record.
  • Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
  • Ability to perform the essential duties of the job.
  • Effectively uses empathy in interactions with others.
