Job Description

The Back Door Mission is seeking to fill the dynamic role of Clinical Engagement Worker. Reporting to the Programs Manager, this is a full-time position, working Monday to Friday from 8:30 a.m. to 4:30 p.m.

The Clinical Engagement Worker takes a case management approach to ensure all clients at the Mission are supported in securing their clinical and health service needs. Working closely with on-site partners, including the Primary Care Clinic, the Clinical Engagement Worker facilitates coordinated care and continuity of services.

In collaboration with hospitals and other healthcare providers across Durham Region, the Clinical Engagement Worker supports smooth and effective transitions for individuals experiencing homelessness as they move between acute care, community-based health services, and the Mission.

The Back Door Mission works to relieve the burdens of poverty and social dislocation within the economically deprived and street-engaged population of Oshawa. Originally a drop-in centre, the Back Door Mission has expanded its services since the COVID-19 pandemic and is currently operating Mission United.

Responsibilities



Review client background information, interview clients to obtain case history and prepare intake reports. Assess clients' relevant skill strengths and needs. Assist clients in sorting out options and developing plans of action while providing necessary support and assistance. Assess and investigate eligibility for social benefits. Refer clients to other social services or assist clients in locating and utilizing community resources, including legal, medical, and financial assistance, housing, employment, transportation, daycare, and other services. Providing a bridge between social services and health care services for those experiencing homelessness. Collecting and reporting on engagement and case management data for program reporting.

Qualifications



Post-secondary education preferred, in healthcare, social services, or related fields Experience working with marginalized populations, specifically those experiencing homelessness and/or mental health and addiction challenges. An understanding of harm reduction principles and trauma-informed models of care. Excellent communication skills, both verbal and written. Experience with Microsoft Office. Ability to maintain a positive, professional, and non-judgmental environment. Experience with conflict management and de-escalation skills. Thorough understanding of social services within the City of Oshawa and Durham Region. Must be willing to obtain a Vulnerable Sector Police check at own cost. CPR/First Aid AED Certification is considered an asset.

Pay range is $24-28/hr, depending on experience with 10 days paid vacation a year and 5 paid wellness days.
