The Job Summary:
The Clinical Dietitian is responsible for providing comprehensive nutritional assessment, care planning and support for residents and patients in Long Term Care and Complex Care Nursing Units. Working as part of the interdisciplinary team, the Dietitian develops individualized care plans, monitors progress and adjusts interventions to support optimal health outcomes. This role also contributes to resident safety, quality of care, and adherence to clinical standards and organizational policies.
Key Accountabilities & Success Criteria:
Assesses residents/patients, identifies problems, risk levels, plans interventions
Develops a nutritional care plan for each resident according to his/her assessed needs.
Documents goals or expected outcomes in the resident plan of care.
Consults with members of the Interdisciplinary Team to determine nutritional needs and therapeutic diets of individual residents/patients.
Assesses residents with dysphagia and makes recommendations for diet modifications
Reviews and updates nutritional assessments quarterly or more frequently if required.
Maintain resident/patient charts and computer data bases in regards to nutritional care.
Attends resident/patient care conferences and family meetings as required.
Advises residents/patients and families on nutrition related issues.
Participates in development of Policies and Procedures for clinical care.
Complies with patient safety programs and processes including identifying and reporting adverse events/near misses
Attends and participates in appropriate safety training or education updates (ie. hand hygiene, emergency code reviews)
Identifies unsafe conditions or practices and takes appropriate action to address them including reporting or removing hazards
Other duties as required.
Qualifications:
Undergraduate degree in Food and Nutrition, or equivalent from an accredited University
Current registration with the College of Dietitians of Ontario
Skills & Abilities:
Knowledge of Ministry of Health and Long Term Care Standards
Knowledge of RAI-MDS coding and documentation
Knowledge of the International Dysphagia Diet Standardization Initiative (IDDSI) recommendations
Previous experience in clinical dietetics, specifically Long Term Care
Computer literacy
Able to work effectively as part of a team or independently
Good organizational and time management skills
Good communication and interpersonal skills with the ability to exercise tact & diplomacy in dealing with residents, other staff and visitors.
