Visit us today and check out to see why more people from across the globe are moving here.About the OpportunityThe Diabetes Management Centres (DMCs) throughout Nova Scotia Health fall under the Primary Health Care portfolio. The DMCs work with primary care providers to provide patients with the support they need to manage their diabetes. The goal of the DMCs are to provide education and self-management support for patients to manage their blood sugars, cholesterol levels, and blood pressure. The Centres assist patients to maintain their health and prevent the complications of diabetes in a model that follows a mix of group, individual and virtual programming. The DMC model advocates and supports an interdisciplinary and integrated approach to diabetes education and management to provide patient centred care.Reporting to the Health Service Manager, the Clinical Dietitian (Diabetes Education) provides extensive disease management support and education to persons who are living with or at risk of developing diabetes. Key components of the role include but are not limited to disease management/care coordination; education; counseling, and program development. The Clinical Dietitian (Diabetes Education) participates in medication management ; insulin initiation; insulin adjustment and follow up; diabetes nutrition counseling; self-management support; physical activity counseling; and providing/facilitating links with resources in the community.
