Job Description

Chancellor Park is currently looking for an experienced Permanent Full Time Clinical Coordinator.

Position:


The Coordinator is responsible for working with the multidisciplinary team to implement and lead the clinical staff and program to ensure the delivery of high quality resident care and services consistent with regulations and established clinical best practices.

The Coordinator has responsibility as a member of the site leadership team to participate in the planning and implementation of strategies and programming that assure high quality resident care while achieving short and long-term operational goals and objectives for the site. The Coordinator provides input in to nursing services, policies and procedures.

The Clinical Coordinator upholds and supports Chancellor Park's mission, vision, values, and care philosophy.

Working Conditions



Will work in collaboration with the other Clinical Coordinators and Resident Care Manager(s) to form the LTC Nursing Leadership team. Regular days of work are Monday-Sunday Regular hours of work are 8:00 am. - 7:00 am. (rotational shifts)

Key Responsibilities:



In collaboration with the nursing management team. The duties of the Clinical Care Coordinator will include at least some of the following:

Supports nursing management team in overseeing Quality Indicators according to LTC policies, provincial and regional regulations, professional colleges, Standards of Practice and which are consistent with Best Practices Working with RAI-MDS. Co-leading the Infection Prevention and Control team. Admitting new residents. Arranging and participating in yearly review of resident care meetings with families. Co-leading required programs: falls prevention, continence care, responsive behaviors, wound care, palliative care, and medication management. Sharing responsibility for quality initiatives, best practices and person-centered care. Continuous Quality Improvement, collection of statistics, audit processes, etc. Assists with orientation of new staff Participates in the evaluation of nursing staff Initiates and/or performs clinical teaching sessions on the units as needed; Works as a unit registered staff member in the event of a critical staff shortage; Completes all other duties as assigned.

Qualifications:



Graduate of an approved School of Nursing; Baccalaureate in Nursing (BN) considered an asset. Current, active registration with the College of Registered Nurses of Newfoundland and Labrador (CRNNL). 5 years' minimum experience in geriatrics, preferably in a leadership capacity. Demonstrated leadership, organizational, communication and interpersonal skills. Broad nursing knowledge. Ability to work with and relate to the interdisciplinary team members and other internal stakeholders, displaying professionalism and employing excellent human relations skills. Ability to work independently and exercise considerable judgment and initiative to complete tasks. Clear aptitude for detail oriented work and strong problem solving abilities. Proficient computer skills.

Salary and benefits are in accordance with Chancellor Park and UFCW Collective Agreement.
