Summary

The Clinical Coordinator, Primary Care, Foundry Granville provides first line supervision, coordinates the day to day operations of the Clinic and ensures that operations comply with the applicable Health Canada requirements. In collaboration with all health care team members, coordinates client flow activities by reviewing clinic processes and developing protocols for improving client flow. Provides clinical direction and coordination of clients and assists the Manager with a variety of administrative activities to ensure efficient and optimal use of client care resources. Liaises with designated team members/staff and supports interdisciplinary practice, quality improvement initiatives and supports interdisciplinary integration of services.

Qualifications / Skills and Education

Education
Registration with the British Columbia College of Nurses and Midwives (BCCNM) as a practicing RN or RPN registrant.

Three years\' recent, related nursing experience in the Community Health Nursing including one year\'s related leadership/supervisory experience or an equivalent combination of training and experience; completion of the Point of Care HIV Testing course; and successful completion of the BCCDC Immunization certificate. CRNBC Sexually Transmitted Infection (STI) Management certification, or evidence of enrollment

Duties and Responsibilities

Ensures staffing levels match client care needs by performing duties such as scheduling and coordinating the use of staff, equipment and other resources on an ongoing basis, assisting staff in workload decision-making, authorizing overtime and providing clinical direction and ongoing monitoring, evaluation and follow-up of work.

Evaluates the provision of direct care to clients and their families in accordance with established standards, policies and protocols and the vision and values of Community Health Services through methods such as assisting staff to operationalize standards, reviewing/auditing relevant documentation, and interviewing clients/families to investigate clinical components of incident reports, provides intervention as required, and ensures care standards are followed. Refers performance related issues to the Manager.

Provides direct health care services to designated clients by performing physical assessments, collecting specimens as necessary, referring clients to the physician for further examination if necessary and administering prescribed treatments and medications.

Ensures clinical prioritization of clients requiring service by allocating staff resources to perform prioritization duties. Communicates with a wide range of health care professionals within and outside the Community health Centre to provide information on the client\xe2\x80\x99s status, arranging access to a variety of specialized services.

Works with the Medical Coordinator and clinic physicians in determining appropriate client/physician assignments and help facilitate the smooth transition from one care provider to another as necessary. Provides input to the Medical Coordinator in ensuring appropriate and safe physician staffing levels.

Monitors and coordinates external and internal client flow through the system with physicians, nurses, front desk staff and other care providers to ensure timely and effective care.

Acts as a resource to staff by consulting with direct care staff on nursing professional practice issues. Collaborates with Community Health Centre Coordinator as required to problem solve issues relating to the daily operations of the clinic and workflow of the clinic. Liaises with front desk staff, physicians and nurses to maintain client flow and ensure safe, timely client care. Promotes a healthy workplace by providing support and input to staff while assisting in establishing clear and consistent guidelines for the processing of clients through the clinic, taking into account staffing levels and client acuity.

Works collaboratively with health care professionals, client/families and community in the identification and resolution of health issues, cultural and language barriers, ethical dilemmas and coordination/integration of care. Makes referrals to other service providers as appropriate.

Provides and/or organizes clinical care, education and counseling using a chronic disease management approach to clients/groups regarding targeted chronic diseases.

Assists the Manager with administrative activities and optimal use of client care resources through methods such as identifying systems for improving utilization of resources, providing input into budget/staff allocation, evaluating client care products/equipment, identifying new equipment purchases required, and recommending changes to existing administrative processes.

Assists the Manager with planning, developing and evaluating client care through methods such as evaluating clinical practice and systems, drafting new and/or revised procedures, standards and criteria, and organizing new clinical initiatives in collaboration with the Physicians, Clinical Educators, Clinicians, and with the interdisciplinary team.

Participates in the selection of new staff and performance evaluation of staff through methods such as attending interviews, assessing clinical skills and providing input. Identifies and recommends training/skill enhancement for staff and participates in staff development activities in collaboration with Manager, Clinical Consultants, Clinician and Clinical Educator, through methods such as conducting needs assessments and coordinating the introduction of new skills/procedures.

Ensures a safe and healthy workplace for clients and staff through methods such as effective orientation, training, promotion and monitoring of safe work practices and enforcement of health and safety requirements. Reports unsafe situations; investigates and takes corrective action and/or notifies appropriate personnel.

Coordinates and participates in quality improvement and risk management activities through methods such as evaluating effect of changes in client care practice, generating recommendations for alternative approaches, following up on incident reports, conducting safety audits and taking corrective action and recommending changes to selected policies, procedures or protocols.

Participates in team meetings, continuing education, conferences and/or corporate initiatives by methods such as providing input from a clinical perspective, presenting relevant materials, acting as an advocate for the goals and objectives of the program and promoting optimal support for clients.

Maintains own clinical currency through methods such as providing nursing care to clients, maintaining involvement in professional/clinical nursing association and conducting regular reviews of related literature and by setting measurable goals and objectives.

Engages in on-going team planning, development, evaluation and quality improvement activities that facilitate team goal setting, problem solving, conflict resolution and collaboration by attending meetings and in-services, providing input/recommendations to the Manager and other team members on efficiency of processes/procedures related to work duties with a view to ensuring streamlined, effective and client/family centred practices.

Performs other related duties as required.
