The Leukemia Coordinator works with the Leukemia Program team & leadership, as well as other internal and external provider care teams to ensure the effective management, planning and coordination of care for Leukemia patients. The principal focus of care will include: management and coordination for patients in shared care, transitioning and planning for allogeneic blood & marrow transplant, and new patient consultations through our Rapid Assessment Clinic. The Leukemia Coordinator will utilize and apply current specialized knowledge and expertise on recent advances in nursing practice to provide, manage, coordinate and facilitate patient care for patients with complex needs both directly and through consultation; functioning as a consultant in the resolution of complex patient management problems; performing teaching/education responsibility; collaborating with the others to develop and implement continuous quality improvement activities; participating in academic and research activities/projects; performing administrative responsibilities where indicated; performing cross-functional and/or other duties consistent with the job classification, as assigned or requested.
