Shalom Village is a non-profit campus of care that specializes in the health and wellbeing of Hamilton area seniors. We provide an unwavering commitment to making every day the best day possible for the seniors who depend on Shalom Village services and programs.

Through their dedication, training and commitment to seniors, our team members transform the lives of older adults through a variety of programs and services, including long-term care, senior living apartments, fitness, community wellness and social programs.

Our long-term care services provide a homey environment and programs for frail seniors, while our assisted living apartments are perfect for more active seniors. Our fitness and community programs support local seniors living at home, while also providing them with the opportunity to socialize with their peers, learn new skills, and stay healthy and fit.

Objective:



Reporting to the Manager of Assisted Living and community Programs, the Clinical Coordinator will effectively manage the day-to-day operations of Shalom Village's Adult Day Program and Goldies 2 Home Adult Day Program, including supervision of staff, students, and volunteers. The Clinical Coordinator is responsible for ensuring high-quality, person-centered care through intake, assessments, program planning, case management, and transitions in alignment with Shalom Village's Strategic and Operational Plans.

The Clinical Coordinator will focus on:

clinical coordination and participant care

program development and oversight

quality assurance

leadership while demonstrating Shalom Village's AT HOME values

Qualifications:



RN diploma or BScN degree Additional certificates in, gerontology, wound care, continuous quality improvement, ?leadership, teaching Excellent computer knowledge and literacy Background in teams: motivating and inspiring others Background in person centered care approaches and quality improvement
Knowledge of PCC, HPG and the RIA-HC.

As a condition of employment, candidates will be required to provide a satisfactory Police Vulnerable Sector Check (PVSC), Two Step tuberculosis screening.

Job Types: Full-time, Permanent

Pay: $31.91-$48.30 per hour

Expected hours: 37.5 per week
