The Clinical Care Coordinator plays a crucial role in ensuring the smooth operation of services within our facility. This position involves coordinating patient care, managing records, and facilitating communication between patients and healthcare providers. This position would serve as a key pillar in ensuring that our mental health services remain accessible, organized, and client-centered as our programs continue to grow.
Key Responsibilities:
Manage the clinical intake process and coordinate client-clinician matching.
Maintain communication with clients pre- and post-therapy, ensuring follow-up, system navigation and payments.
Provide administrative support to social workers and Director-: scheduling, progress tracking, and coordination with external supports.
Track client outcomes and service usage to assist with reporting and strategic planning. Maintain accurate, confidential records in coordination with clinicians.
Follow up with therapists to ensure payments are made in a timely manner
Support the AMRC clinical team and reflect a commitment to holistic, client-centered care that addresses the full scope of a person's needs.
Work closely with other AMRC staff to complement other services
Qualifications:
Knowledge of Resources:

Community Education and Experience:
Post-secondary education in
Social Work

Knowledge of
