About the Role
The Client Care Coordinator (RPN/RN) plays a key role in ensuring exceptional client experiences by leading care consultations, overseeing care quality, and supporting CAREGivers in the field. This is a
non-hands-on nursing position
focused on assessment, coordination, relationship-building, and quality assurance rather than direct personal care.
You will be a trusted clinical presence for clients and families, helping them navigate care decisions while ensuring services remain aligned with evolving needs.

Key Responsibilities
Conduct
Care Consultations
with potential clients and families to assess needs and recommend personalized plans of care
Serve as a clinical liaison and trusted advisor to clients, families, CAREGivers, and referral partners
Coordinate and oversee
client/CAREGiver introductions
to ensure smooth care starts
Complete
Quality Assurance visits and follow-ups
to ensure high standards of care and client satisfaction
Continuously evaluate and adjust care plans based on client needs and feedback
Build and maintain relationships with community referral partners (retirement homes, LTC, hospice, hospitals)
Collaborate closely with internal team members to support cohesive, high-quality care delivery
Maintain accurate documentation and client records within the operating system
Support CAREGiver engagement and retention initiatives, including recognition programs
Participate in weekly team and client care meetings
Travel throughout the Ottawa, Kingston, Belleville, and surrounding areas as needed (mileage reimbursed)
Schedule & Work Environment
Monday to Friday, 8:30am-5:00pm
One Saturday per month
Occasional evenings to accommodate Care Consultations
Community-based role with travel across the exclusive territory
Qualifications
Valid
RPN or RN license
in good standing with the College of Nurses of Ontario (CNO)
Valid driver's license and reliable vehicle (kilometer reimbursement provided)
Strong communication, assessment, and relationship-building skills
Comfortable working independently while collaborating with a team
Organized, detail-oriented, and confident in clinical decision-making
Passion for community care and supporting clients outside of institutional settings
Reporting Structure
Reports directly to the
Director of Care
