Job Description
We are seeking a reliable and compassionate Client Care Coordinator to join our team. This role supports clients, families, and care staff to ensure high-quality home care services are delivered smoothly and efficiently.
Key Responsibilities
Coordinate and monitor client care services. Communicate effectively with clients, families, and staff. Ensure accurate scheduling and documentation. Support quality, safety, and client satisfaction. Collaborate with the care team to resolve issues promptly.
Qualifications
Diploma or degree in Health or Social Services (or equivalent experience). Strong communication and organizational skills. Professional, detail-oriented, and client-focused. Flexibility for occasional evening or weekend support.
