Position Summary

Coordinated Service Planning (CSP) is a seamless and family-centered service experience for families of children/youth who have multiple and/or complex special needs. Quinte Children\'s Treatment Centre (QCTC) is the lead agency for CSP in Hastings, Prince Edward, and Northumberland Counties. Numerous agencies in these regions have partnered to offer CSP through the creation of the HPEN Network Partnership. Families of children/youth with special needs often find it difficult and complicated to navigate multiple services, making it challenging for them to ensure they are choosing the best plan for their child. CSP is the process of bringing people together to talk about the strengths, interests, hopes, priorities, and goals of a child/youth/family with multiple and/or complex special needs. The Coordinated Service Planner also facilitates the creation of a living document that grows and develops with the child/youth, to help ensure that the service providers involved with a child/youth work collaboratively to support the child/youth\'s ability to participate fully at home, at school, in the community, and as they prepare to achieve their goals for adulthood.

Required

  • Must be a graduate of a college diploma in Social Service Work
  • Member in good standing in the College of Social Workers of Ontario,
  • Three years\' experience working in the children\'s service sector, preferable with children with special needs and their families,
  • Demonstrated awareness of community resources for families and children/youth with complex special needs in Hastings, Prince Edward and Northumberland Counties,
  • Experience and understanding of a Family Centred Care model of service delivery,
  • Knowledge in the development of SMART goals,
  • Solution-Focused Coaching" is considered an asset (or similar)
  • Demonstrated leadership skills in facilitating the development of a Coordinated Service Plan,
  • Excellent communication skills and proven ability to work as part of a team with professionals, support staff and other community agencies,
  • Strong organizational skills and ability to multi-task,
  • Ability to work hours that accommodate family schedules,
  • Valid driver\'s license and ability to travel across the service area,
  • Ability to travel out of town for provincial collaboration, if needed,
  • This position is primarily in-centre. Approval of any work from home will be at management discretion and may change over time.
DESIRABLE QUALIFICATIONS:
  • Ability to support francophone children/youth and families,
  • Ability to support the cultural needs of indigenous children/ youth and families.
Duties
  • Assist the child/youth and/or family in identifying their visions and goals,
  • Lead families and team members in the development of a strengths-based Coordinated Service Plan for children and youth with multiple complex needs, based on family/child goals,
  • Provide input into and follow the community protocol related to Coordinated Service Planning process,
  • Explore flexible and innovative approaches for service delivery to meet the needs of the child/youth,
  • Facilitate the exchange of information between relevant providers in the children\'s services, education and health sectors and ensure their involvement in the Coordinated Service Plan,
  • Monitor, review and update the Coordinated Service Plan, in collaboration with the family and relevant providers as the child and family\'s needs and services change,
  • Provide mentorship to community team members regarding the protocol and development of a Coordinated Service Plan,
  • Participate in local and provincial Communities of Practice related to Coordinated Service Planning,
  • Gather statistical data as per Centre and ministry requirements,
  • Document all service involvement as per Centre, ministry and college requirements,
  • Adhere to all privacy legislation related to consent and the sharing of personal health information,
  • Support continuous quality improvement,
  • Consistently demonstrate optimism, warmth, empathy, collaboration/support of clients and colleagues, continuous learning/improvement, willingness to take direction, and a strong work ethic,
  • Take a child- and family-centred, strengths-based approach founded on the F-Words for Child Development,
  • Support families in a way that is culturally safe, responsive, and promotes equity, anti-oppression, anti-ableism, and anti- racism, including dismantling anti-Black and anti-Indigenous racism,
  • Acts as a resource and support to community service providers in the practice of CSP,
  • Work collaboratively with children/youth/families, other team members and community partners to ensure a seamless, streamlined and integrated service experience,
  • Source and/or collaboratively develop appropriate educational and resource materials for children/youth/families,
  • Participate in QCTC interdisciplinary teams, clinics, working groups and committees,
  • Other duties, as assigned.
Quinte Children\'s Treatment Centre (QCTC) is one of 21 Children\'s Treatment Centres in Ontario, providing a range of child development and rehabilitation supports for children and youth with special needs and their families. QCTC is part of Quinte Health, with segregated funding received from the Ministry of Children, Community and Social Services. The centre offers a range of services to support child development and rehabilitation for children and youth with special needs, and their families. Services include rehabilitation supports (Occupational Therapy, Physiotherapy and Speech Language Therapy) for preschoolers and school-aged children in home, school, and community settings, Social Work, Psychometry and Medical Services, System Navigation, Coordinated Service Planning, and Fetal Alcohol Spectrum Disorder Workers. QCTC is an Autism Diagnostic Hub as well as a SmartStart Hub - a clear entry point to service for families and caregivers who are concerned about their child/youth\'s development, and their day-to-day functioning related to development.
