The Respite Coordinator is responsible for working with families, respite providers, Ministry personnel, professional supports and community agencies, with a primary goal of facilitating respite arrangements for parents. The role will also focus on improving the quality and consistency of existing programs and expanding the number of respite opportunities provided by CLTO, including development, implementation, and support of out-of-home respite services for children.
Qualifications
Bachelor's degree in Social Services, Developmental Services, Human Services or a related discipline. Additional training including Abuse Prevention Reporting and Rights of a Child, Prevention, Abuse, through Counselling and Training (PACT), First Nations, Inuit and Metis (FNIM) and Supporting Individual Rights. Applied Behavioral Analysis (ABA) training is an asset. Minimum 2 - 3 years of experience working with children and/or adults in a respite setting.
Strong knowledge of Autism Spectrum Disorder (ASD) and applied behavior analysis. Knowledge of Developmental Services Sector.
Proficiency in computer software including Microsoft Office (Word, Excel, PowerPoint, Outlook). Familiarity with medications, Safe Management Group (SMG)/Crisis Prevention Intervention (CPI).
Familiar with Ministry of Children, Community and Social Services polices and/or procedures and strong understanding of the Child, Youth and Family Services Act.
Familiar with community tables and respite services. Strong problem-solving skills to resolve any issues/concerns brought forward by families. Ability to build strong and trusting relationships with caregivers. Advocacy and sensitivity to diversity for families and children. Able to work effectively as part of a team. Strong verbal and written communication skills. Detail orientated. Ability to develop and maintain strong community alliances with community agencies, partners, and child and youth service programs across the region.
Driver's license is an asset as role involves frequent travel through the region.
Responsibilities
Intake - Receives referrals from various services and serves as first point of contact for all the families referred to the CLTO Respite Program. Manages new referrals, completes tours of the respite programs and gathers relevant information about the service user from family; Social Workers or Service Coordinators Attends home visits if necessary. Explains program, forms, schedule, applied behavioural analysis (ABA), medication; provides information, answers questions and plans respite visits. Completes all aspects of intake package with families (e.g., registration, Plan of Care (POC), consent forms) and maintains all documentation (e.g., enters information into the ISIS client database, files intake package and information as per policy, maintains in-service spreadsheet; provides monthly statistics for the Ministry for the Respite programs, ensuring guidelines and targets met. Completes Safe Bed intake as required.
Scheduling - Manages/assists with scheduling for booking respite, observation visits with individuals, consults with internal interdisciplinary team where applicable Organizes, prioritizes, and maintains a manageable schedule for the individuals and their families. Communicates with families, support networks (family support coaches, social workers, and service coordinators) on a regular basis to ensure respite is being accessed. Provides and receives feedback on respite visits with families and service providers. Maintains schedule, informs site of staffing needs and person specific preparations required. Records in service days to reflect service vs. target numbers. Schedules school visits when required. Organizes summer camp when required; handle purchasing of supplies, snacks, special diet foods, items for activities.
Community - Participates in various committees, liaises with external stakeholders as needed, serves as point of contact for various crisis networks, attends inter-disciplinary team meetings (internal and external) when required, liaises and makes recommendations at community planning tables for respite.
Brings forward all the concerns/issues raised by families; works to solve problems, and if necessary, involves Program Supervisor, Behaviour Services Consultant, and Manager. Researches and establishes links with emerging and promising practices, ensures external respite websites information is up to date, applies for Flex Funding when necessary and submits invoices to recover funds, ensures Discharge Reports are completed and forwarded to appropriate stakeholders at the end of respite. Updates ISIS client database and SharePoint site. Assists with transition planning to other children and adult respite services internally. Completes survey with families.
CLTO - Responds to Respite Choices inquiries, tickets and logs requests and updates their status. Maintains SharePoint for Respite Group/Respite Choices website; creates and maintains the Respite Choices Resource Guide, Choices Facebook page and Respiteservices.com information. Attends information fairs. Provides respite options/supports and services for families, updates Respite Choices Information sheets and respite forms, reviews and monitors the agency's respite intake process and provides support to programs, and manages intake for children's Safe Beds program and provides support to staff and supervisors for all CLTO respite programs.
CLT March Break Camp - Prepares and promotes the March Break Camp; distributes information and applications. Gathers all required intake packages, completes selection process, follows up with families and confirms camp details, informs camp designate of the participants, scans and forwards forms to Shadow Lake Centre, ensures targets are met, enters information into the ISIS client database, completes attendance report for billing and surveys with families.
