Job Description

JOB PURPOSE/SUMMARY



Provide in-home one on one support for children and caregivers to support the health and wellness of families and develop and strengthen life skills abilities as well as to connect to appropriate services both in and outside of the community. To deliver culturally relevant, trauma informed child wellbeing services that supports families/community and ensures the safety and well-being of children. Develop and maintain healthy relationships with external partners and community resources.

QUALIFICATIONS



Education:



College Diploma - Social Services or related field or relevant experience. Bachelor Degree - Social Work an asset.

Skills and Abilities:



Strong communication and counselling skills. Conflict Resolution and ability to effectively address complex situations. Knowledge of Child & Family Services Act. Excellent interpersonal skills and the ability to work collaboratively with internal/external community partners. Knowledge and understanding of the historical impacts of child welfare practices and the ability to deliver services through a trauma informed lens. Must have a valid Class G driver's licence. First Aid/CPR and Infectious Disease Control Training to be completed within first 3 months of employment.

Experience:



Front line social service experience with a strong emphasis on child welfare. Experience working with court documents. Experience and understanding of social work practice rooted in Indigenous culture. Experience in coordinating and facilitating programs and workshops. Providing counselling - 1-3 years experience.

KEY JOB FUNCTIONS/RESPONSIBILITIES



Able to affirm to the vision, mission, and guiding values of Wewena Minodoodawin Health & Social Services, and the Chippewas of Rama First Nation by demonstrating commitment to personal growth and wellbeing and the ability to develop healthy relationships that encompass compassion and dignity with sensitivity to issues related to poverty, homelessness, mental illness, addictions, developmental disabilities, ensuring a strengths based, trauma informed approach to care. Establish and maintain healthy, therapeutic relationships with children, youth, and families, assessing their needs and providing in home support as required. Develop and maintain respectful and cooperative relationships with all service organizations that contribute to the delivery of integrated and seamless services for Rama First Nation children, youth, families and community. Work with families in their home as a preventative approach to provide wholistic supports, education and strategies to build successful outcomes. Assess appropriate support services for families through a trauma informed lens. Advocate and assist families with system navigation (education, social, health, legal, etc.) and connection with appropriate services both in and outside of the community to support increased family health and wellbeing. Liaise regularly with MKES Attendance Worker to support increased school attendance Provide on-going supports and services to families to ensure service plans/wholistic plans are relevant and meeting their needs. Assist team with strength-based parenting programs and provide individualized parenting programs as required Attend Case Management meetings as required to provide updates and input regarding family wellness plans Ensure strict confidentiality at all times and only within the client's circle of care. Keep up-to-date, accurate case notes utilizing EMH-Ware. Regular reporting/debriefing to supervisor/Manager. Provide monthly reports and statistics and an annual roll-up. Other duties as assigned.
