Key Qualifications:

Valid

Personal Support Worker (PSW) Certificate

(required) Fluent in

spoken and written English, and fluent Tagalog

Computer literate

with basic documentation and reporting skills Strong

time management

and organizational abilities Skilled in

weekly meal planning

and cooking

healthy meals

Certified in

Medication Administration

Holds a valid

Food Handling Certificate

Proven experience working with clients with

dementia

Friendly, social, and able to build strong rapport with clients Must be

accountable

, mature, and safety-conscious

Key Responsibilities:

Provide

personal care

and support with daily activities (bathing, grooming, dressing, toileting)

Administer medications

safely and monitor for reactions or health changes Prepare and serve

nutritious meals

tailored to the client's dietary needs Maintain a

clean, hygienic, and safe home environment

Support with

mobility and transportation

, including escorting to appointments Encourage and engage in

social interaction

to enhance emotional well-being Keep accurate

documentation

and communicate with healthcare professionals as needed Assist in

coordinating services

and managing care plans

Experience:

Previous experience in

home care, senior support, or assisted living

required Understanding of

chronic condition management

(e.g., dementia, diabetes, dialysis) Familiarity with

hospice or palliative care
