Qualifications

Graduation from an accredited school of nursing and possession of a valid license.

Actively registered and in good standing with their professional college

Work in accordance to the legislation as outlined by the Health Professions Act (HPA).

Provide annual verification of registration and good standing with regulatory body

Current HCP CPR

Mandatory Covid 19 Vaccinations

Experience working with mental health clients preferred.



Under the supervision of the Director of Care, the LPN will manage all care and assessed services for the Clients and the facility, within their scope of practice. The LPN will also provide direction and supervision to the HCA's, Undergraduate Nurses, Recreation Aides and cleaning staff at the facility. Duties are performed in accordance with established policies and procedures and in a manner which respects the uniqueness, dignity, worth, need for self -respect and, self-determination of the Client all in ways; which promotes maximum personal and functional independence as a result of holistic care.

Responsibilities

The LPN's responsibilities include but are not limited to observation, responding to situations, documentation, reporting, follow-up and promoting the quality of life for Clients of EPINSS.

The LPN will be:

Responsible for the provision of the emotional and physical comfort of clients

Encouraging Clients to participate in completing their Activities of Daily Living as noted on the Care Plan This is inclusive of personal hygiene, ADLs, and incontinent care. Where the Client is unable to complete their care, the LPN will assist as needed.

Participating in providing consistent environmental cleanliness- client rooms, common areas, office, bathrooms, etc.

Supervising direct support staff (DSS) to ensure these tasks are performed properly.

Observing, assessing, supporting, recording, and reporting on the condition of a Client with changing health care status This may include, but is not limited to:

physical changes (abilities, vital signs, appearance, signs of shortness of breath, pain, skin integrity, change in mobility)

Behavioral changes not consistent with their established Care plan.

These observed changes are to be reported as soon as possible to the DOC for further direction.

Documenting in a chronological sequence inclusive of observations, treatment initiated, outcome of the treatment and evaluation. If the situation is assessed as an emergency by the LPN and/or the DOC, 911 is to be called. Appropriate documentation must be collected to provide to emergency personnel.

Responsibly Managing Client medications by:

Administering medications to clients according to the physician orders and EPINSS policy,

Reducing the risk of medication error and potential harm by using the 8 rights of medication administration -right patient/Client, right drug, right dose, right route, right documentation, right time, right reason, and right to refuse.

Storing all medications appropriately according to medication information and pharmacy direction,

Documenting administration time, occurrence, and effect accurately,

Reporting medication errors immediately and providing medical support as needed and directed by physician,

Following proper safe medication disposal procedure,

Ordering medications as per physician's orders in a timely manner

Participating in narcotic counts at beginning and end of shift and ensuring all narcotics are accounted for.

Providing ostomy care, wound care, and catheter care, inclusive of consistent assessment, reporting of changes, and supporting the involvement of specialties as needed and approved by the DOC

Assisting with the rehabilitation of clients in accordance to their care plan.

Participating in the training and supervision of HCAs to learn to provide care for the Client according to their care plan.

Assisting in gathering information for the care plan, providing insight into the needs of the Client and implementation of the care plan objectives.

Coordinating Medical Appointments by:

Scheduling all doctor and dental appointments for the Client.

Informing the DOC of all Client emergency room visits and hospitalizations.

Providing current and accurate medical information to authorized personnel, as needed.

Following up to ensure all procedures, orders and recommendations have been addressed within 24 hours of a medical appointment. (The overnight LPN on Thursdays will follow up from Wednesday's on-site clinic.)

Completing all documentation of medical visit outcomes.

Undergraduate Nurses acting in a supervisory role shall inform EPINSS of their Scope of Practice and notify the DOC of their limitations in any practices required of the LPN
