Title: Case Worker

Reports To: Manager of Programs & Services

Summary of Position

The Case Worker supports individuals experiencing homelessness by providing client-centered case management services aimed at securing housing stability, increasing income, and improving overall well-being using a team management approach. This role involves conducting assessments, facilitating access to community resources, and maintaining accurate and timely documentation. Case Workers play a key role in fostering trust, promoting self-sufficiency, and ensuring clients are supported with dignity, compassion, and respect.

Education

Degree or diploma in relevant discipline (e.g. Social Work, Psychology, Social Service Work, etc.)
Other Requirements

Valid Driver's License with clean Driver's Abstract Working knowledge of Loss and Separation, Family crisis, Poverty, Disability, Unemployment, Resettlement, Homelessness, Addiction and violence issues. Knowledgeable on local community resources
Skills Required

Certifications and Skills: CPR/First Aid, Non-Violent Crisis Intervention, HIFIS (Homeless Individuals and Families Information System), Rent Smart, Mental Health First Aid. Understanding of Homelessness: Working knowledge of the causes of homelessness in the region. Mental Health Expertise: Knowledge of concurrent mental health disorders, their treatment, available resources, and recovery approaches. Naloxone Knowledge: Familiarity with Naloxone and its role in reversing opioid toxicity. Problem-Solving Abilities: Strong problem-solving skills, utilizing analysis and creativity to address unique challenges. Health & Safety Advocacy: Understands and promotes Health & Safety as a shared responsibility.

Duties & Responsibilities

Conduct client intakes, assess immediate needs, and complete VI-SPDAT assessments to determine housing and support priorities. Develop individualized case plans that reflect each client's goals related to housing, income stability, health, and personal well-being. Attend and coordinate client appointments, including medical, legal, and housing-related engagements; accompany clients when needed. Facilitate warm hand-offs and maintain collaborative relationships with community service providers, housing workers, and external agencies. Monitor client progress through regular follow-up and goal tracking, adjusting plans as circumstances change. Maintain accurate, up-to-date, and professional documentation in HIFIS, including case notes, action plans, and outcome reporting. Provide crisis intervention and emotional support while maintaining healthy boundaries and trauma-informed practice. Participate in interdisciplinary team meetings and case conferences to coordinate care and ensure holistic client support. Educate clients on their rights and responsibilities as tenants and help build life skills needed for sustainable housing. Uphold strict confidentiality protocols and ensure all records and interactions meet legal and ethical standards. Maintain a safe, welcoming, and respectful environment that prioritizes client dignity, autonomy, and cultural sensitivity. Contribute to program development by identifying service gaps and recommending improvements to better support client outcomes. Responsible for providing on sight support and oversight to ensure smooth daily operation and staff coordination in the absence of management or team leads.

Pay: $45,700.00-$45,760.00 per year
