Position Purpose summary:
The Case Worker is responsible to provide residents with appropriate case management support services using a holistic approach thus ensuring that the mental, emotional, social, physical and spiritual needs are met.
Accountabilities:
To provide case management and support services to residents of MMC Residence using the CTI model. To provide the following services as required:Individual case management to alleviate mental, emotional, and social needs providing the appropriate level of support during someone journey of change Needs assessments and follow-up (Case Assessment tool and Outcomes Star) Advocacy Crisis intervention Referrals to appropriate programs/agencies Liaise with professionals in the community Liaise with residential workers Discharge planning Assist residents' integration into community housing Follow-up as necessary To maintain accurate detailed documentation/case notes relative to specific case load. To provide adequate appropriate information to other staff with regards to residents' needs and management by means of logs, case conferences, team meetings, and other vehicles as deemed appropriate. To actively participate in case conferences, team meetings, staff development, staff and community meetings as required To be actively involved in supervision with the Program Service Manager on a regular basis. To maintain records and statistics as prescribed by Administration

CRITICAL RELATIONSHIP MANAGEMENT:
Governance Boards and Councils: None Internal:Reports directly to the Program Service Manager Develop close working relationships with all staff within the organization including

Housekeeper, Janitors, Counsellor, Housing Worker, Community Follow-Up Worker, Chaplain, Administration and the Management Team
External:Establish and maintain good working relationships with external agencies to make the appropriate referrals.

MANAGERIAL/Technical Leadership RESPONSIBILITY:Reports directly to: Program Service Manager

WORKING CONDITIONS:This is permanent full-time position based on 40 hours per week. Works in a shelter environment There may be frequent interruptions and multitasking Equipment and general office noise (i.e. people talking, phones ringing) May interact with clients with erratic/aggressive behaviors May deal with angry and/or abusive people May deal with verbal abuse

WORK ENVIRONMENT
The incumbent's work environment is typically in the entire facility and surrounding property designated. The Incumbent will work under the following disagreeable conditions:May interact with clients with erratic/aggressive behaviors May deal with angry and/or abusive people May deal with verbal abuse Odors May deal with bodily fluids May encounter sharps, safe drug supplies May deal with onsite pest infestations (bed bugs etc)

EXPOSURE TO HAZARDS
The incumbent may be exposed to the risk of moderate to severe injury or illness due to:Physical attacks by others Exposure to infectious diseases Exposure to toxic chemicals Moving Objects Sharp Objects (i.e., needle pricks) Electrical Hazards Repetitive strain injuries (e.g., back, wrist)

PRECAUTIONARY MEASURES
The incumbent must take the following precautionary measures (in addition to health and safety rules):Universal precautions Use gloves Use ear/eye protection Eye wash stations Fume masks where appropriate Appropriate face mask (either medical or N95 and would be based on IPAC protocols)

HEALTH AND SAFETY RESPONSIBILITIESComply with the provisions of the Occupational Health and Safety Act and Regulations Adhere to the agency's Health and Safety Program Report accidents/injuries/workplace hazards in a timely manner Conduct facility security/safety rounds on a regular basis according to policy Adhere to all Health and Safety Policies of the agency

EDUCATION, QUALIFICATIONS AND CERTIFICATIONS
Education, Qualifications and Certifications:Bachelor of Social Work (BSW) or equivalency or Counselling Diploma plus 2 years' experience Standard First Aid and C.P.R. Certificate Crisis Prevention and Intervention (CPI) Certificate preferred

Experience and Knowledge Requirements:Previous experience working with homeless population (one to three years) Significant experience and demonstrated skill in working with people with mental health and addictions issues Must be comfortable visiting clients in their homes and in the community Knowledge of Residential Tenancies Act Good knowledge of community resources Knowledge and understanding of, and a commitment to anti-racism and anti-oppression

Skills and Capabilities:Excellent empathetic listening and client focused assessment skills Strong oral and written communication skills (computer skills essential) Demonstrated ability to take initiative and work independently as well as part of a team Valid Driver's License would be an asset (no vehicle required Vulnerable Sector Check required

Compensation:
The target hiring range for this position is $31.42 with a maximum of $32.21.

The expected Work Hours - 8:00am - 4:30pm - Monday - Friday
Compensation:
The target hiring range for this position is $31.42 to $31.82 with a maximum of $32.21.
Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
