Position Summary and Scope of Service:

Reporting to the Manager, Autism Case Management and consistent with the Mission, Values and policies of St.Amant, the Case Worker is a critical role in ensuring the success of St.Amant business and operations.
Exemplifying the mission, vision and values of St.Amant, the Case Worker works collaboratively with partnered programs while offering employees respectful customer service.
The Case Worker is primarily a remote position with most meetings and services offered virtually, and some services provided in-person in the client's home and/or in the community on an as needed basis. The Case Worker provides case management services for children with Autism Spectrum Disorder. The Case Worker is expected to work as a member of a service delivery team and use sound independent judgement. The Case Worker uses the professional application of social work theory and methods to deliver discipline-based services and intervention strategies to individuals, families, agencies and care providers through the following steps of the case management process:
Referral and intake
Crisis and/or protection services
Assessment (including risk assessment)
Case planning, counselling and support services provision, including financial management
Service monitoring and evaluation
Service termination
The Case Worker will require in-depth knowledge of program policies, directives and applicable legislation. The Case Worker will take an active role in promoting service and developing creative strategies to work with families and systems. The Case Worker promotes a positive, nurturing environment for the family and works toward strengthening support networks, enhancing knowledge, competencies, skills, and family stability using a family-centered approach within an ecological model of practice.
The Case Worker provides services in accordance with the Mission and Values of St.Amant, policies and procedures of Clinical Services, external legislation and regulatory bodies and all relevant professional standards of practice.
The Case Worker is familiar with and promotes a mindful, family-centered, customer service focus in the provision of services to clients, staff and in other professional relationships.
The Case Worker promotes processes that contribute to self-determination and the development of skills and competencies of individuals and care providers. This pertains to all areas of intervention including assessments, treatment, and implementation of service.
The Case Worker is responsible for evaluating effectiveness and efficiency of interventions to ensure outcomes that are positive and beneficial to clients
The Case Worker is familiar with social policy, issues, trends, best practices in the field, needs, gaps or opportunities which may improve the lives of persons with Autism Spectrum Disorder, or which promote social justice, equity or accessibility.
The Case Worker is familiar with and adheres to all policies related to the program, risk management and those of workplace activity, promotes and follows all safety, health and respectful workplace practices.

Qualifications
Educational:
Must possess a Bachelor of Social Work (BSW) Degree from an accredited university
French language skills are an asset
Professional:
Must be eligible for and maintain registration with the Manitoba College of Social Workers including meeting all prescribed continuing education requirements of the profession
Minimum of two years' social work experience in the area of developmental disabilities is preferred
Clinical:
Demonstrated knowledge and experience using a family-centered model of service provision
Knowledge of the history of services to people with disabilities, current trends, philosophies and practices concerning their care, education and lifestyles
Knowledge and experience working with children and families and experience working in the area of developmental disabilities/Autism Spectrum Disorder
Knowledge and ability to function within the legislation and regulations pertinent to practice, including CFS Act, Vulnerable Persons Legislation, and Personal Health Information Act
Expertise in conducting social/family assessments using an ecological framework and family-centered approach
Knowledge of psychosocial and environmental factors involved in chronic disability which may impact family functioning or adjustment, and the ability to define appropriate intervention strategies
Ability to form appropriate professional/trust relationship with clients
Demonstrated counseling skills including supportive, adjustment, and crisis counseling
Ability to assess and identify risk level of situations and respond appropriately
Working knowledge of service systems and ability to creatively access or develop informal and formal supports or resources
Ability to document plans and interventions accurately and in a timely manner, using standardized program protocol and formats
Ability to communicate openly and appropriately using positive interpersonal communication skills
Knowledge of the dynamics inherent in the client-worker relationship and skillful in preparing for transition and in determining the nature of support required upon termination
Ability to develop and conduct specialized group interventions and to document benefits and outcomes
Ability to organize, chair and conduct meetings
Ability to understand, support and practice interdisciplinary teamwork in order to provide services in the clinical, educational or living milieu of the person with Autism Spectrum Disorder
Ability to maintain an active caseload, providing assessment, service plan development, intervention planning/implementation and service outcome evaluation
Ability to work within direct, mediated and consultative models of practice
Understand own clinical limitations and seek support and consultation for clinical activities so as to do "no harm"

Duties and Responsibilities
Program/Service Support
Consistent with the Mission, Philosophy and Policies of St.Amant, the Case Worker is directed toward family empowerment, is cognizant of the individual needs of each family member, and recognizes that they are part of a much broader community and social system. Quality services are evident when professional communication, timeliness, accuracy, ethical awareness / sensitivity, confidentiality are consistently demonstrated as required by legislation and program/service standards.
Duties include:
Receives and reviews new referrals
Maintains familiarity with referral sources in the general community
Provides social assessments using an ecological framework and family-centered model in order to facilitate and develop plans which specify goals and outcomes
Performs initial risk assessment as required
Empowers families to develop formal and informal supports to facilitate the most effective and efficient utilization of resources and to enhance coping skills
Ensures clinical outcomes, internal and external services and supports are monitored, documented and shared appropriately and effectively
Investigates all incidents of abuse/neglect consistent with applicable Acts, Regulations and prescribed standards
Recognizes and prioritizes high-risk indicators or potentially abusive situations; reports and documents all high risk or potentially high risk situations according to policy and legislation, proceeds with intervention or follows up to prevent further occurrences
Coordinates/facilitates the resolution process related to family concerns or dissatisfaction and provides mediation and conciliation services
Participates as needed in team meetings or service review meetings; ensures service plan and timelines are specified for clinical intervention
Responsible for initiating Age of Majority planning as the individual approaches adulthood
Ensures that a transition plan is identified, that timelines are specified, and that the family is transitioned appropriately on termination of services
Exercises statutory responsibilities in the event of removal of an individual
Arranges emergency placement as warranted
Consults and collaborates with various service streams (police, school, legal guardian/decision-makers, mental health, hospitals, identified resources, etc) to arrange critical services
Provides crisis services throughout case life as warranted
Strategizes services for complex/high-needs individuals and families
Identifies individual/family/agency/care provider strengths and needs
Interprets and analyzes case information and recommends appropriate resources
Assists in the creation of a case plan identifying/recommending service requirements
Evaluates if services and supports are meeting desired outcomes of case planning goals
Ensures ongoing required supports are in place from other systems as required
Provides education or information, verbal and/or written, with respect to services and resources
Facilitates and attends groups or meetings in response to identified need with both internal and external supports
Consults effectively with other supports and services to ensure coordination of planning
Responds to emergency/crisis situations in a timely manner
Completes required administrative functions when closing files
Maintains internal/external partnerships and works collaboratively and strategically on multidisciplinary teams
