Description: Youth Transitional Housing provides youth aged 16-25 a program to stabilize and change their lives by providing a youth-driven goal focused program to become self-sufficient.
This program works with youth who face a number of barriers. We work with each youth at their starting point, helping them achieve their goals and ambitions as they journey toward independence. Our work is youth focused, based on the principles of harm reduction, resiliency, and trauma -informed, solution focused practice, and a strength-based approach.

Build rapport with youth and engages in active listening
Facilitate case assessments and case management sessions with youth.
Support youth in developing client-centered goals and monitoring their goals and progress.
Encourage youth in working towards goals and action steps (school, life skills, job, rehabilitation etc.)
Communicate progress of the youth, changes in behavior, any concerns/challenges that youth are facing with team members and legal guardians.
Provide referrals and follow-up support to external service providers, community and other various stakeholder groups.
Manage and maintain all documentation to ensure appropriate file management procedures are followed.
Qualifications:
Bachelor's Degree in a related field or two-year Youth Care Worker Diploma including trauma-based care.
Human Services Certificate.
Experience working in a youth residential facility would be a strong asset.
Must have excellent verbal and written communication skills and a clean drivers abstract.
All candidates will be required to have a valid driver's licence, a reliable vehicle and provide a clear criminal record and social development checks.
Strong interpersonal, non-judgmental, empathetic, flexible and ability to work with initiative and independence
Extensive background and experience working with youth struggling with addictions, poverty, mental health, and trauma.
Working knowledge and experience with harm reduction, resiliency, trauma-informed and solution focused practice.
Excellent written and verbal communication skills.
Proficient and skilled with computer use and database entry
