Position Title: Case Manager, Full-Time Term Position (Female Home)
Hours: 80 Hours Bi-Weekly
Wage Range: $21.68 - $25.40 per hour
Schedule: Primarily daytime hours (8:00 AM - 4:00 PM); flexibility required to meet operational needs

Case Manager
Positions Summary
The Case Manager provides support, direction, assistance, and advocacy services to the House Residents, in accordance with the mission and philosophy of United Church Halfway Homes.
The Case Manager is to be a leader in giving direction, support and delegating responsibilities to other staff regarding the care of residents.
Duties and Responsibilities with House Residents
Provide thorough case management and support to the residents of the House
Manage a caseload of up to 10 residents
conduct comprehensive resident assessments to develop a case plan for each resident
ensure support systems are in place to meet resident needs by identifying and coordinating a variety of available services necessary to maintain independent living, self-sufficiency and family stabilization
monitor and ensure resident compliance with their plan
conduct crisis intervention as necessary
give supportive counsel to residents and refer them to available therapeutic, social service, educational and medical resources as needed
assist residents to access external services and /or advocate where necessary to help residents access the services or resources they require
provide assistance to residents to establish and maintain appointments
encourage, plan and attend with residents recreational and /or social activities, performing duties such as introducing residents to community resources
transport individuals as required or needed to appointments, case conferences and parole/probation meetings
assist residents with budgeting their funds, planning their purchases
assist residents in preparation of meals to learn basic cooking skills
support efforts to meet the spiritual needs of residents by way of demonstrated interest and participation in programming activities where necessary
ensure resident chores are assigned and complete
Duties and Responsibilities with Agencies
maintain frequent contact with the placement agencies (CSC, WRHA, PASS and so on) regarding specific residents and to establish relationships
attend parole, probation and case conferences held by the placement agency
be familiar with placement agency procedures and policies and remain current with any changes
plan for and attend in-reach events in correctional facilities
be familiar with agency services in the community and attend networking meetings
be familiar with other halfway services in the region
participate and assist with outreach events
Duties and Responsibilities with other Clients
participate in urinalysis collection and administration of paper work and reporting as appropriate
support the House Manager in making decisions regarding residents, including deciding on acceptance and departure
review potential residents through OMS and WRHA and other provincial systems
conduct new resident's intake and ensure all supporting documents are verified and on file
maintain case management files for residents
complete all required reports for CSC, WRHA, PASS and other placement agencies
ensure documentation is completed and appropriately filed when a resident leaves
coach Community Reintegration Facilitators and provide opportunities for them to participate in case management processes
provide leadership in reviewing residents atstaff meetings
inventory and distribute medications, when necessary
participate in shopping for groceries and other supplies as directed
participate in preparing meals as directed
assume the role of Acting House Manager, if prepared to do so, when requested
Other duties as assigned by the House Manager or Executive Director
Reporting
The Case Manager reports to the House Manager
Qualifications
minimum of two years' experience in case management services
education from a college or university preferred
training and/or understanding of Mental Health/Axis 1 disorders
experience working with criminalized men/women recently released from correctional institutions, psychiatric wards or hospitals
ability to meet deadlines and be self-motivated
motivated with sense of team work and ability to work independently
excellent communication in English, both written and verbal
effective people skills
knowledge of community resources
demonstrated knowledge of indigenous cultural traditions and practices
demonstrated skills in working with people from various backgrounds

Demonstrated proficiency in MS Word Excel, PowerPoint and internet usage
Ability to pass an Enhanced Security Risk Clearance through Correctional Service Canada
