Position Overview
: We are seeking a compassionate, organized, and dedicated Case Manager to join our team at Village Family Health Team. The Case Manager will be responsible for coordinating and providing services to individuals in need, assessing client needs, developing care plans, monitoring progress, and advocating for resources to ensure client well-being and goal achievement
.
As a member of our team at VFHT, you will play a pivotal role in delivering high-quality care to our patients. You will work closely with other healthcare professionals (physicians, psychiatrists, nurse practitioner, social workers, nurses, chiropodist, pharmacist, dietician, physiotherapist, and administrative team) to assess, plan, and provide comprehensive care to meet the needs of our patient population.
Responsibilities:
Conduct intake assessments to determine client needs, goals, and eligibility for services
Develop, implement, and monitor individualized service or treatment plans
Field visits, helping and accompanying to visits if needed
Coordinate referrals and link clients to appropriate community resources and services
Advocate on behalf of clients with service providers, government agencies, and other stakeholders
Maintain detailed, timely, and accurate case documentation in accordance with organizational and legal requirements
Monitor client progress and adjust care plans as needed
Collaborate with internal teams and external partners to provide holistic support
Ensure compliance with all confidentiality laws, regulations, and ethical standards
Participate in team meetings, training sessions, and case reviews
Maintain accurate and up-to-date electronic health records (EHR) in compliance with regulatory standards.
Qualifications and Requirements:
Bachelor's degree in Social work, Social services, Nursing, Psychology or a related field
[1-3] years of experience in case management, social services, or a related field
Knowledge of local community resources and support systems
Excellent communication, interpersonal, and problem-solving skills
Ability to manage a caseload effectively and prioritize competing demands
Proficiency in documentation
Ability to work both independently and collaboratively in a team-based care model.
Ability to work in multiple sites and with partner organizations.
